3.0 CERTIFICATION OF TRUE COPIES OF DOCUMENTS

CHECKLIST

 

1. The document - Original & photocopies as required. Must be marked properly with post-it page markers.

* Non-Malaysian documents must, first - be certified true copies by the issuing country's Consulate / Dept. of Foreign Affairs.

2. Applicant’s MyKad, passport and current-valid Visa Grant Notice & VEVO.

3. Processing fee: Aud5.00 per page, cash & exact only.

4. Processing time: Three (3) working days.

5. This service is limited to certain types of document & purposes only:

- i.e. Malaysian / non-Malaysian documents for use in / outside of Malaysia.

- Please contact the Consulate should you require detailed clarification.

 

Note: Authentication / Verification of original Malaysian documents can only be done at the Ministry of Foreign Affairs in Putrajaya, Malaysia (www.kln.gov.my).

 

4.0 APOSTILLE ON AUSTRALIAN DOCUMENTS

CHECKLIST

 

a) If the document is issued by non-government agencies, it must have:

- Verification stamp by an Australian Notary Public and

- Authentication stamp from the Australian Dept. of Foreign Affairs & Trade.

b) If the document is issued by government agencies, it must have:

- Authentication stamp from the Australian Dept. of Foreign Affairs & Trade.

c) The Consulate may only do apostille on Australian documents that have the stamp(s) as above.

2. Applicant’s MyKad, passport and current-valid Visa Grant Notice & VEVO.

3. Processing fee: Aud5.00 per apostille, cash & exact only.

4. Processing time: Three (3) working days.

5. This service is limited to certain types of document & purposes only:

- i.e. Australian documents for use in Malaysia.

- Please contact the Consulate should you require detailed clarification.

 

5.0 RENUNCIATION OF MALAYSIAN CITIZENSHIP

CHECKLIST

 

1. Applicants must be over twenty-one (21) years of age.

2. Form K, duly filled on three (3) original copies at the Consulate.

3. Original & three (3) photocopies of:

- MyKad, front & back.

- Birth certificate.

- Malaysian passport (latest)

- Malaysian citizenship certificate.

- Australian passport.

- Australian citizenship certificate.

- Australian change of name / identity certificate (if applicable)

4. Passport-sized photo, white background - Three (3) nos.

5. Applicants must surrender the original:

- MyKad.

- Malaysian birth certificate.

- Malaysian passport (latest)

- Malaysian citizenship certificate.

6. Application fee: Aud10.00 cash & exact only.

7. Processing time: Six (6) months minimum.

8. Malaysian birth certificate will be returned with a Confirmation of Renunciation.

 

6.0 ATTESTATION (WITNESSING OF SIGNING OF DOCUMENTS)

CHECKLIST

 

a) The attestation will be signed by a Consular Officer - Please check with your lawyer / agent to ensure this is acceptable for your matters.

b) The Consular Officer's capacity does not extend to explaining the content of your legal document and / or the court procedure.

c) Please obtain clarification from your lawyer / agent on where you / the Consular Officer needs to sign & mark the locations with post-it page markers.

 

1. The document - Must be marked properly with post-it page markers.

2. Applicants must produce the following:

- MyKad.

- Passport.

- Current-valid Visa Grant Notice & VEVO.

- Black ink-pen as specified by the lawyer / agent.

3. Processing fee: Aud5.00 per signature, cash & exact only.

4. Processing time: Three (3) working days.

5. This service is limited to certain types of document & purposes only:

- e.g. Malaysian S&P / property / legal docs., statutory declarations, KWSP & JPJ forms.

- Please contact the Consulate should you require detailed clarification.

 

7.0 TRANSLATION OF MALAYSIAN DOCUMENTS

CHECKLIST

 

Translation service by the Consulate is not NAATI Certified - Please check with your lawyer / agent to ensure this is acceptable for your matters.

 

1. The document - Original & one (1) photocopy.

2. Applicant’s MyKad, passport and current-valid Visa Grant Notice & VEVO.

3. Processing fee: Aud10.00 per licence / page, cash & exact only.

4. Processing time: Three (3) working days.

5. This service is limited to certain types of document & purposes only:

- e.g. Driving licence, JPN’s birth / marriage certificates & SPM / PMR / UPSR results.

- Please contact the Consulate should you require detailed clarification.

 

8.0 EXTRACTION OF MALAYSIAN BIRTH CERTIFICATES

CHECKLIST

 

1. Application form, duly filled on two (2) copies, 2-sided:

a) For births in Peninsular Malaysia, Borang JPN.LM12

2. Applicant’s MyKad, passport and current-valid Visa Grant Notice & VEVO.

3. Original & two (2) photocopies of the Child's (as in the birth cert.):

- MyKad, front & back (or foreign nationals ID card if not a M'sian citizen)

- Passport.

- Birth certificate (if any)

- Other pertinent documents.

4. Provide telephone number & e-mail address in the application form.

5. Processing fee: Aud10.00 per certificate, cash & exact only.

6. Processing time: Four (4) months.

 

9.0 VERIFICATION OF DEATHS OF A MALAYSIAN CITIZEN

CHECKLIST

 

a) Registration of deaths is to be done directly with the National Registration Department (JPN) of Malaysia.

b) For detailed information regarding the procedure / requirement of registration please contact the JPN in Malaysia or visit www.jpn.gov.my/en/core-business/death

c) The Consulate’s role is limited to verifying the death documents and issuance of an official letter subject to the following:

 

1. Death certificate from Births, Deaths & Marriages Reg. Office.

** Must have the following information:

- Name, date & place of birth of the deceased.

- Date, place, cause & certifier of death.

- Cremation / burial / repatriation information.

** Must have authentication stamp on the back page from the Australian Dept. of Foreign Affairs & Trade.

 

2. Reporting person’s MyKad, passport and current-valid Visa Grant Notice & VEVO.

3. Original & two (2) photocopies of:

- Death certificate.

- The deceased's MyKad, front & back.

- The deceased's Malaysian passport.

- The deceased's Australian passport (if any)

- The deceased's birth certificate (if any)

- The deceased's change of name / identity certificate (if applicable)

4. Processing time: Three (3) working days.
