REGISTRATION OF BIRTH PROCEDURE, MALAYSIAN BORN ABROAD

1. Malaysians born abroad can be registered at any Malaysian Mission on the condition:-

i) Father of the child is a Malaysian citizen

ii) Registration is made within one year from the date of birth

iii) Date of birth / age

iv) Address in home country

v) Address of relative or next of kin that can be contacted

2. Please bring the child upon registration at the Consulate and documents required are as follow:-

a.	Passport and Identity card (ID card) of parents
b.	Original Birth Certificate of Parents and Chinese Notary of Birth-Certificate for Chinese citizens which has to be bilingual and attested by the Ministry of Foreign Affairs Office (FAO) of the Country or Province of issue.
c.	Malaysia Marriage Certificate
d.	Child’s Original Birth Certificate issued by the local hospital
e.	Notarized certificate of Child’s Birth which has to be bilingual and attested by the Ministry of Foreign Affairs Office (FAO) of the Country or Province of issue
f.	Three (3) recent passport-sized photographs with white background
g.	Application ‘Form D’ JPN.WN02 for Birth Registration Note : Registration according to the name in the birth certificate and translation / Pendaftaran nama anak mengikut nama seperti sijil kelahiran.
h.	All original documents for verification and 3 sets of photocopies (a-f) of the following documents:

3. Fees for submission of application ‘Form D’ and for issuance citizenship certificate ‘Borang H’ is RMB65.00

Note: -

For registration made beyond the period of one (1) year after the birth process 1-3 still applies. The Consulate will forward the application to the National Registration Department of Malaysia for approval. Feedback of the application (approved or otherwise) will be notified to the applicant.

The child born in Malaysia can be a Malaysian if either parents (father or mother) is a citizen of Malaysia.

REGISTRATION OF MARRIAGE PROCEDURE

Malaysian citizens who wish to marry in CHINA need to understand that the Local Marriage Registration Offices have their own procedures and requirements.
Malaysian citizens who are wish to marry in CHINA please contact the China Marriage Registration Office in advance for details. The Chinese Marriage Registration Office in SHANGHAI can be contacted at No. 82 Cao Bao Lu, Shanghai or telephone number (8621) 64325087.
To register marriage - please make sure all required documents are complete and make an appointment in advance. The documents for marriage registrations are:

a.	Original Passport and Identity card (ID card) husband and wife.
b.	Malaysia Birth Certificate for Malaysian and Chinese Notary of Birth-Certificate for Chinese (attested by Respective Foreign Affairs Office).
c.	China Marriage certificate issued by the local Marriage Bureau
d.	Notarial certificate of marriage which has to be bilingual and attested by respective Foreign Affairs Office (FAO) of the Province of issue of Marriage Certificate.
e.	Three (3) copies recent passport-sized photographs for husband and wife with a white background
f.	Form JPN.KC06 (Fill this form during appointment).
g.	All original documents (a – d) need to make three (3) sets of copies of the following documents.

Marriage certificates issued by any country which is not in English must be translated to English and attested by the Ministry of Foreign Affairs of the country of issue, and Embassy / Consulate General of Malaysia / Consulate of the respective country.
Malaysian subjects MUST re-register with a Consulate General Office within 6 months from the date of marriage, otherwise a penalty of CNY100.00 will be imposed from the first year of marriage and CNY50.00 for every subsequent year. Fees for marriage registration is CNY44.00.
