Purpose

This document is issued to Malaysian Citizens under circumstances such as in cases of lost passport abroad, persons who have been repatriated to Malaysia on Government expenses and for Malaysian in case where Malaysia Passport cannot be issued within the stipulated period.

Applicant must come personally to the Embassy. Applicant may also contact the Honorary Consulate General of Malaysia in Barcelona.

Documents required to report a lost or stolen passport are as follows:

1 Full police report

2. A copy of completed and duly signed lost passport application form (IM.42c)/ Borang soal selidik kehilangan pasport dan akuan sumpah. *

3. A copy of completed and duly signed passport application form (IM.42) * + Lost passport report.

4. Original and one (1) photocopy of original MyKad and Birth Certificate

Note: If the original MyKad/ Birth Certificate is not available in the UK and is in Malaysia, please seek assistance from your next of kin in Malaysia to have the original MyKad/ Birth Certificate (currently in Malaysia) to be certified true copy at any National Registration Department or Immigration Department in Malaysia. A certified true copy is the photocopy of a MyKad/ Birth Certificate that is stamped and signed as a true copy of the original by an authorized National Registration or Immigration officer. Subsequently have the certified true copy scanned and email/ posted/ fax to the Embassy. Please do not take the risk to post the MyKad/ Birth Certificate as it might get lost in postage.

5. Original and one (1) photocopy of original Citizenship Certificate (if applicable)

6. One (1) photocopy of lost or stolen passport (if any)

7. Four (4) passport photographs.

8. Proof of currently studying at a university/ education institution - e.g: Cover letter with university’s letter head (including address and contact details) confirming that the student is currently studying at the university. The cover letter has to be recently signed by a managerial/ human resource/ administrative personnel of the university. For Government sponsored student, confirmation letter has to be issued by the sponsors (e.g: MOHE, JPA, MARA, Petronas or Bank Negara).

9. Proof of dependent to a work permit or student visa holder - e.g: Letter from sponsor or photocopy of spouses’ work permit or student visa.

Note:

An Emergency Certificate will be issued by the Embassy which would enable the person to travel home to Malaysia (one way trip).

APPLICATION TO RENOUNCE MALAYSIAN CITIZENSHIP

Applicant must come personally to the Embassy.

Documents required are as follows:

i. 2 copies of Application Form K JPN (Perkara 23-Kaedah 36), duly filled

ii. Malaysian Passport (original and photocopy)

iii. Malaysian Identity Card (original and photocopy)

iv. Malaysian Birth Certificate (original and photocopy)

v. Malaysian Citizenship (if any)

vi. Letter of Naturalisation with translation in English (original and 2 photocopies)

vii. 3 passport size photographs

viii. Fee: EUR 3.00

CERTIFICATION OF MALAYSIAN DOCUMENTS (CERTIFY TRUE COPY)

Applicant must come personally at the Embassy.

Documents required are as follows:

i. Documents (original and photocopy)

ii. Passport (original and photocopy)

iii. Malaysian Identity Card (original and photocopy)

iv. Please require 2 working days

v. Fee: EUR 5.00 each document

AUTHENTICATION OF DOCUMENTS

Application can be made in person at the Embassy.

Documents required are as follows:

Note: Malaysia is not member of The Hague Convention, please legalize all documents to the Ministry of Foreign Affairs and Cooperation Spain before its submission to the embassy.

Los documentos requeridos son los siguientes:

REGISTRATION OF DEATH

Application can be made at the Embassy.

Documents required are as follows:

i. Original Passport of the deceased

ii. Original Birth Certificate and Identity Card of the deceased

iii. Copy of the Death Certificate

iv. Copy of the Embalming Certificate

v. A copy of the Police report is required

REPATRIATION OF REMAINS

The relatives/ next of kin in Malaysia reports at the Consular Division, Ministry of Foreign Affairs at the following address:

Information and documents required are as follows:

i. Name of the deceased

ii. Passport and Identity Card Number

iii. Address of the deceased abroad

iv. Address in Malaysia

v. Occupation

vi. Cause of death

The Consular Division, with the assistance of the respective Malaysian Mission abroad will investigate the death.

The relative/ next of kin of the deceased will need to decide the place of burial (abroad or in Malaysia).

BURIAL IN MALAYSIA

The Consular Division, with the assistance of the respective Malaysian Mission abroad will obtain the estimated cost of the repatriation which will have to be deposited by the relative/next of kin to the Consular Division. A receipt will be issued.

The respective Mission will carry out the repatriation process.

The Consular Division will notify the family on the flight details for the arrival of the remains.

BURIAL ABROAD

The Consular Division will instruct the respective Embassy to coordinate the burial process.

All cost incurred will have to be borne by the relative/next of kin which will have to be paid to the Consular Division. A receipt will be issued.

The Consular Division will submit the following documents to the relative/next of kin:

i. Acknowledgement letter from the respective Mission

ii. Certificate of Health

iii. Certificate of Death

iv. Certificate of Burial

v. Death Report

vi. Copy of Passport/ Identification Card (original document will be submitted directly to the respective departments in Malaysia)

viii. Last known address of the place of work (if available)

ix. Name and address of family to be contacted in Malaysia

x. Name, address and telephone number of the person making the report/ government agency involved

xi. Reasons for going abroad

The Consular Division will seek assistance from the respective Embassy to investigate the case.

The family will be informed of the result of the investigation.

If the missing person has been found, the family will have to bear the cost of sending the person back home (flight ticket and other costs incurred). The money needs to be deposited to the Consular Division and a receipt will be issued.

The family will be informed of the details of arrival. The family will have to wait to receive the person at the airport or other entry points in Malaysia.

ARREST/ DETENTION BY LOCAL AUTHORITIES

Relative /family/ acquaintance makes a written report about the arrested/detained person at the Consular Division:

Information required for the reports are as follows:

i. Name of the arrested/detained person

ii. Passport and Identity Card number

iii. Date and place of arrest/detained

iv. Telephone number of the place of arrest/detained

v. Cause of arrest/ detention

The Consular Division will seek assistance from the respective Embassy to investigate. The family will be notified of result of the investigation.

If the family requires legal assistance or need to visit the arrested/ detained person, the application needs to be submitted to the Consular Division. All cost incurred will be borne by the family.

After completion of sentence (if found guilty), the person will be deported back to Malaysia. In some countries, cost of deportation will be borne by the country concerned. In others, the family needs to bear the cost.

Deposit the required amount to the Consular Division. A receipt will be issued.

The family will be informed of the details of arrival. The family will have to wait to receive the person at the airport or other entry points in Malaysia.

REGISTRATION OF ABSENTEE VOTERS

Registration of voters for Malaysians abroad is divided into 2 categories:
Postal Voters / Absent Voters consists of :

i)         Arm Forces who are in service
ii)          Spouses to (i)
iii)         Government personnel (Federal Government, States Service or Local Authorities) who are in service abroad
iv)        Spouses to (iii)
v)         Individuals studying full time in Universities, Technical Colleges, Training Institutions or other bodies abroad (only if they are sponsored by the Government)
vi)        Spouses to (v).
Normal Voters :
Malaysian other than the above Malaysian Missions abroad will make the announcement for the registration and checking of the voting list.

Registration can be made at the Embassy by filling in a form provided. Address, telephone number and e-mail is available in the main page of this website.

Among documents required for the registration are:

-           Identification document that confirms the applicant's citizenship
-           Identification Card
-           Passport
-           3 passport size photograph

For the election, normal voters need to return to Malaysia to cast their votes based on the area they have registered in. For postal voters, casting of votes are made by post

GOOD CONDUCT CERTIFICATE

The application could be made online at the Ministry of Foreign Affairs's website: http://www.kln.gov.my and click:

e-Consular under Online Services.

2. The completed application form must be printed and submitted with supporting documents within two (2) weeks from the date of application through counter or via post to the following address:

i. Application from Malaysian citizen/non-citizen in Malaysia

Applications must be sent to the respective offices of Malaysian Missions overseas.

The documents required are as follows:

1. Two (2) copies of the duly completed application form for the Certificate of Good Conduct;
2. Two (2) certified copies of identification card;
3. Two (2) certified copies of passport namely pages that contain details and signature of the applicant;
4. Two (2) recent passport-sized photographs; and
5. Two (2) copies of Statutory Declaration, except for applicant who will be visiting any of the following countries: The United States of America, Australia, Singapore, Spain, France or Papua New Guinea.

6. Copy of residence permit, if applicable

7. Fee, Euros 6.00

CONSULAR INSCRIPTION CERTIFICATE

Issuance of this certificate is based on record of REGISTRATION OF MALAYSIAN OVERSEAS.

Application can be made in person at the Embassy or via email.

Documents required are as follows:

i. Original passport, 2 photocopies of main page & observation page

ii. Birth Certificate or Malaysian Identity Card (original and 2 photocopies)

iii. Valid Spanish Residence Permit

iv. 2 recent passport photographs size 3.5 cm x 5.00 cm

v . Fee: EUR 6.00

REMINDER:

Malaysians visiting The Kingdom of Spain are advised to observe the 90 days free

visa period and not to abuse the facility by attending other activities prohibited on a visitor

visa or overstaying. Those who wish to work, study or remain in Spain are advised to obtain

relevant entry clearance from the Spanish authorities. Please contact Embassy of Spain in

Malaysia for travel formalities (www.maec.es).
