REGISTRATION OF CHILDBIRTH OVERSEAS (WITHIN ONE YEAR)
(UNDER ARTICLE 14 OF THE FEDERAL CONSTITUTION - CONFIRMATION OF CITIZENSHIP) FORM D (CERTIFICATE ISSUED: FORM H)
 
1. Eligibility:
1.1. The father must be a Malaysian citizen.
1.2. The child must be registered within one (1) year from the date of birth.
 
2. Both parents and the child must be present at the High Commission. Please bring the original and two (2) photocopies of the following documents. All photocopies must be made on A4-sized paper.
 
2.1. Child's Documents:
a. Three (3) recent passport-sized photos with a blue background.
b. UK Birth Certificate, showing details of both parents (must be legalised by the UK FCDO).
c. Personal details page of British or any foreign passport (if available; must be notarised by a UK Notary Public and subsequently legalised by the UK FCDO).
 
2.2. Parent's Documents:
d. Malaysian Marriage Certificate.
e. MyKad (photocopies of the front and back must be made on the same page).
f. Malaysian Birth Certificates.
g. Personal details page of valid Malaysian passport and valid UK Visa with share  code.
h. If the mother is not a Malaysian citizen:
  • UK Birth Certificate must be legalised by UK FCDO, otherwise (please see notes).
  • British citizen — Personal details page of valid passport (must be notarised by a UK Notary Public and subsequently legalised by the UK FCDO).
  • Non-British citizen — Personal details page of valid passport (must be notarised by a UK Notary Public and subsequently legalised by the UK FCDO) and valid UK Visa with share code.
 
 
Notes:
  • A copy of any non-Malaysian document must be notarised by a UK Notary Public and subsequently legalised by the UK FCDO.
  • Documents not originally in English or Malay must be translated into English. Copy of the original document and the translation must be notarised by a UK Notary Public and subsequently legalised by the UK FCDO.
  • If legalisation cannot be completed within the UK: The document must be legalised by the Foreign Office of the issuing country and subsequently legalised by the Malaysian Embassy in that country.
 
 
3. Fees:
i. £10.00 — Registration and issuance of the certificate (Form H).
ii. £5.00 - Per legalisation.
 
4. Processing Time:
At least four (4) weeks after submission of complete documents.
 
 
REGISTRATION OF CHILDBIRTH OVERSEAS - LATE REGISTRATION FORM D
 
1. General Information:
1.1. For children registered MORE THAN ONE (1 ) YEAR after the date of birth, application will be sent to Jabatan Pendaftaran Negara (JPN), Malaysia for processing and approval.
1.2. The father must be a Malaysian citizen.
1.3. Official letter of explanation from the father regarding the delay.
1.4. The Consular Division will not be responsible for any errors on the certificate if the mistake occurred during the completion of the original form by the applicant or processed by JPN.
 
2. Both parents and the child must be present at the High Commission. Please bring the original and two (2) photocopies of the following documents. All photocopies must be made on A4-sized paper.
 
2.1. Child's Documents:
a. Three (3) recent passport-sized photos with a blue background.
b. UK Birth Certificate of the child, showing details of both parents (must be legalised by the UK FCDO).
c. Personal details page of British or any foreign passport (if available; must be notarised by a UK Notary Public and subsequently legalised by the UK FCDO).

APPLICATION FOR MALAYSIAN CITIZENSHIP FOR A CHILD BORN OVERSEAS - WHERE THE MOTHER IS A MALAYSIAN CITIZEN AND THE FATHER IS A FOREIGN NATIONAL 
UNDER ARTICLE 15(2) OF THE FEDERAL CONSTITUTION 
 
1. General Information:
1.1. This application applies to a child born abroad where the mother is a Malaysian citizen and the father is a foreign national.
1.2. Application must be submitted by the mother.
1.3. Subject to Ministerial approval.
1.4. The Consular Division will not be responsible for any errors on the certificate if the mistake occurred during the completion of the original form by the applicant or processed by JPN.
 
2. Both parents and the child must be present at the High Commission. Please bring the original and two (2) photocopies of the following documents. All photocopies must be made on A4-sized paper.
 
2.1. Child's Documents:
a. Three (3) recent passport-sized photos with a blue background.
b. UK Birth Certificate of the child, showing details of both parents (must be legalised by the UK FCDO).
c. Personal details page of British or any foreign passport (if available; must be notarised by a UK Notary Public and subsequently legalised by the UK FCDO).
 
2.2. Sibling's Documents of child:
d. Malaysian Birth Certificate (if available) or UK Birth Certificate must be legalised by UK FCDO, otherwise (please see notes).

REGISTRATION OF CHILDBIRTH OVERSEAS - UNMARRIED MOTHER (SINGLE MOTHER) FORM D
 
1. General Information:
1.1. Application will be sent to Jabatan Pendaftaran Negara (JPN), Malaysia for processing and approval.
1.2. Please note that for a single-parent application, the father's details on the child's birth certificate must be left blank.
1.3. The mother must sign Statutory Declaration Act 1835 (must be notarised by a UK Notary Public and subsequently legalised by the UK FCDO) for children over one (1) year old.
1.4. The mother must sign the Surat Akuan, which will be provided at the counter, in the presence of a Consular Officer.
1.5. The Consular Division will not be responsible for any errors on the certificate if the mistake occurred during the completion of the original form by the applicant or processed by JPN.
 
2. The mother and the child must be present at the High Commission. Please bring the original and two (2) photocopies of the following documents. All photocopies must be made on A4-sized paper.
 
2.1. Child's Documents:
a. Three (3) recent passport-sized photos with a blue background.
b. UK Birth Certificate of the child. (must be legalised by the UK FCDO).
c. Personal details page of British or any foreign passport (if available; must be notarised by a UK Notary Public and subsequently legalised by the UK FCDO).
 
2.2. Sibling's Documents of child:
d. Malaysian Birth Certificate (if available) or UK Birth Certificate must be  by UK otherwise (please notes) 
e. Personal details page of passport (if available; please see notes).
 
2.3. Mother's Documents:
a. MyKad (photocopies of the front and back must be made on the same page).
b. Malaysian Birth Certificate.
c. Personal details page of valid Malaysian passport and valid UK Visa with share code.

LETTER OF ISSUANCE FOR OVERSEAS MUSLIM MARRIAGE DOCUMENTS
 
1. General Information:
 
This application is intended to verify and attest to the relevant marriage documents and certificates obtained in the UK, strictly for the purpose of facilitating your official marriage registration at the State Islamic Religious Department (Jabatan Agama Islam). Please be advised that you are required to register your marriage within six (6) months of the date of marriage.
 
2. Please provide the following documents. All photocopies must be made on A4-sized paper.
 
a. MyKad (photocopies of the front and back must be made on the same page).
b. Personal photo page of valid Malaysian passport and valid UK Visa with share code.
c. If the spouse is not a Malaysian citizen:
  •  British citizen — Personal details page of valid passport (must be notarised by a UK Notary Public and subsequently legalised by the UK FCDO).
  •  Non-British citizen — Personal photo page of valid passport (must be notarised by a UK Notary Public and subsequently legalised by the UK FCDO) and valid UK Visa with share code.
d. Original UK Muslim Marriage Certificate (must be notarised by a UK Notary Public and subsequently legalised by the UK FCDO). The UK Muslim Marriage Certificate must clearly state the details of the issuing organisation and the venue where the marriage was solemnised.
e. Conversion Certificate (if applicable; please see notes).
 
Notes:
  • A copy of any non-Malaysian document must be notarised by a UK Notary Public and subsequently legalised by the UK FCDO.
  • Documents not originally in English or Malay must be translated into English. Copy of the original document and the translation must be notarised by a UK Notary Public and subsequently legalised by the UK FCDO.

3. Provide A4 prepaid self-addressed envelope with full name as per passport with complete UK residential address.

REGISTRATION OF NON-MUSLIM MARRIAGE OVERSEAS
 
1. General Information:
 
Under Section 31 of the Law Reform (Marriage & Divorce) Act 1976, non-Muslim Malaysian citizen who have married abroad and registered their marriage under the civil law of a foreign country, they are required to re-register their marriage within six (6) months either at the nearest National Registration Department or Malaysian Mission.
 
2. Both spouses must be present at the High Commission to complete one (1) copy of Form KC06. The original form must be signed in the presence of a Consular Officer.
 
3. Please bring original and two (2) photocopies of the following documents. All photocopies must be made on A4-sized paper.
 
a. Two (2) recent, professionally taken passport-sized photos with a white background.
b. MyKad (photocopies of the front and back must be made on the same page).
c. Personal details page of valid Malaysian passport and valid UK Visa with share code.
d. Malaysian Birth Certificate.
e. UK Marriage Certificate (must be legalised by the UK FCDO).
f. Malaysian Decree Absolute (if applicable).
g. If the spouse is not a Malaysian citizen:
• British citizen
- Birth Certificate (must be legalised by the UK FCDO).
- Personal details page of valid passport (must be notarised by a Notary Public and subsequently legalised by the UK FCDO).
- UK Decree Absolute (if applicable; must be notarised by a UK Notary Public and subsequently legalised by the UK FCDO).
 
• Non-British citizen
- Birth Certificate (please see notes).
- Personal details page of valid passport (must be notarised by a UK Notary Public and subsequently legalised by the UK FCDO) and valid UK Visa with share code.
- Decree Absolute (if applicable; please see notes).
