Any reports of missing person (Malaysia) abroad do so at the Consular Division, Ministry of Foreign Affairs Malaysia at the following address:

(b)   Place/date of birth
(c)   Details of Malaysian Passport
(d)   Identification Card number
(e)   Date of departure from Malaysia
(f)    Date of arrival in the particular country
(g)   Subject’s full address and telephone number abroad (if available)
(h)   Last known address of workplace (if available)
(i)    Name and address of family/next of kin to be contacted in Malaysia 
(j)    Photograph of subject (if available)
