2.	Renunciation of citizenship	Covering letter from applicant; Malaysian Identity Card / MyKad (original and copy -photocopy of both side); 2 form K; Malaysian Passport (original and copy); Original Malaysian Birth Certificate; and 2 recent passport size photographs (in blue background).	Rs.200
3.	Verification of Citizenship	Covering letter from applicant; Applicant’s Birth Certificate (original & 2 copies on A4 size paper); Parent’s Birth Certificate (original & 2 copies on A4 size paper); Malaysian Identity Card (applicant & parent) (original & 2 copies on A4 size paper); If the mother is not Malaysian Citizen then her passport will be used as identification Parent’s Certificate of Citizenship (original & 2 copies on A4 size paper); Parent’s Marriage Certificate (original & 2 copies on A4 size paper); Parent’s Entry Permit (original & 2 copies on A4 size paper); Passport of the applicant & parents (all pages) (original & 2 copies on A4 size paper); 3 passport size photographs of the applicant; Letter School (if schooling) (original & 2 copies on A4 size paper);	Rs.30		Applicant must be present / come in person (for the thumb print)
4.	Attestation of Documents	Covering letter from applicant; and Document(s) to be attested. the document(s) must first being attested by Ministry of External Affairs	Rs.200 per attestation	2 working days	Documents for attestation must be submitted at Visa Application Counter @Gate 2 (8.30 am – 10.00 am)
5.	Emergency Certificate (EC)	Copy of the passport; Malaysian Identity Card / MyKad (original and copy); First Information Report / Police Report; Passport Application Form (IMM.42-Pin.1/97); 3 recent passport size photograph (in light blue background)	Rs.1000	2 working days
6.	Application for an Extract of Birth / Marriage / Death Certificate	Covering letter from applicant; Passport (original and copy); Malaysian Identity Card / MyKad (original and copy - photocopy of both side); and 1 Extract of Birth / Marriage / Death form.	Rs.200

PHOTOGRAPH REQUIREMENTS

1. Passport size;

2. 35mm wide by 45mm high;

3. Without border;

4. Taken within the last 3 months;

5. Full face without headgear;

(Unless the applicant habitually wears a headgear in accordance with his / her religious or racial custom but the headgear must not hide the applicant’s feature.)

6. The facial image must be from chin to crown; and

7. White background (for Visa) & light blue background (for Passport / Emergency Certificate).

* Scanned photos are not at all acceptable. If photograph found to be scanned, visa will be rejected.
