1.	Valid passport with more than 6 months validity beyond date of entry into Malaysia and a photocopy of the information page
2.	The applicant must be a resident of Sweden, Denmark, Norway, Iceland. A photocopy of the Residence Permit endorsement in country of residence has to be submitted
3.	Two (2) identical passport photographs
4.	Introduction letter from Employer or College, and date resuming for duties/studies in country of residence has to be stated too. If you are having your own business, a copy of the business license has to be submitted
5.	Invitation letter from Malaysia (if any) to be faxed directly to this Embassy together with a copy of the sponsor´s Malaysian Identity Card, (if the sponsor is a Malaysian). If the sponsor is not a citizen of Malaysia, a copy of the sponsor´s passport and work/ residence Permit has to be submitted instead
6.	Latest Bank Statement showing last 3 months of transactions. (Proof of applicant´s financial ability to finance the stay in Malaysia)
7.	Confirmed return ticket or onward journey to the third country and a photocopy. The flight ticket must show the period of stay allowed in Malaysia to enable Stockholm’s Office to issue visa. For the period of stay allowed for different citizens, please check on the website www.jimlondon.net or www.kln.gov.my. Any extension of stay is at the discretion of the Immigration Officer in Malaysia.
8.	Completed and signed two (2) sets of application forms (IM47 – Pin 1/97) Forms can be downloaded from websites, imi.gov.my or jimlondon.net (download application forms)
9.	Visa fee including postage fee with registered mail, with acknowledgement slip (AVI) and all payments are to be paid in cash in Swedish Crowns. The visa fees are charged based on the citizenship of the applicant and may fluctuates due to the monthly exchange rates from Malaysia and therefore please call the Embassy or Honorary Consulate in your country of residence for the up to date fees. The postage fee for the registered mail for a 100g (i.e. 1 passport) envelope for the year 2006 is SEK77.00 (within Sweden) and SEK104.00 (outside Sweden). For applicant who wishes to enclose stamps, please send only international stamps (from outside Sweden). Usually the post office in Sweden will increase their fee every year from the beginning of each year and therefore it is advisable to call us for the up to date fees before sending the application.

1.	Please complete two (2) copies of Borang K and submit with the following documents either in person or mail to our Honorary Consulate or to:-

5.	Your original Malaysian Citizenship certificate (not applicable if you are born after 31.08.1957) and a photocopy.

NOTE :

1.	The "Borang "K" (K Form) is available at the Embassy/Honorary Consulate. You may collect it personally or send us a self-addressed prepaid 100g envelope if you want it to be mail-delivered to you.
2.	If you are unable to come to the Embassy or the Honorary Consulate to sign the forms in front of an Officer, you may ask the Commissioner of Oath, Notary Public or a Lawyer to witness both your signatures and thumbprints. Please make sure that the name of the Officer who witnessed your signature is printed/written clearly
3.	If any of the mentioned document(s) cannot be produced, a Police Report in English made by the applicant should be forwarded, proving report of any loss, damaged or theft of documents. If it is impossible to get the Police Report in English, please get it translated first before submitting the Report to the Embassy.
4.	Please be informed that application will be sent to the National Registration Department, Malaysia (Jabatan Pendaftaran Negara) for verification and approval and the process may take 6 months to a year. One form “K” and your cancelled Malaysian Birth Certificate will be returned to you after approval of your application for your retention.

Please be informed that this Embassy is NOT an authorized translator. However the Embassy may translate the Malaysian document(s) such as the Birth Certificate, Marriage Certificate, Identity Card and Driving License from Bahasa Malaysia to English only.

The fee for the translation for each document is SEK100.00 and legalisation is SEK100.00. Fees are to be paid in Swedish crowns in cash and receipt will be issued accordingly.

IMPORTANT:

Couple that have married outside Malaysia must register their marriage with the nearest Embassy, within six (6) months from the date of marriage. Late registration of marriages exceeding six (6) months will have to pay a penalty of RM$100.00 for the first year and thereafter RM50.00 per year. All payments are to be made in Swedish crowns in cash and receipts will be issued accordingly.

NOTE :

1.	The Marriage Registration Application Form (JPN.KC 06) is available at the Embassy/Honorary Consulate. You may collect it personally or send us a self-addressed prepaid envelope if you want it to be mail-delivered to you.

FOR CATEGORY 2,

With effect from 1^st June 2010, a Malaysian mother is able to register her child´s birth.
Embassy will forward the application to the National Registration Office (JPN), Malaysia for approval.

The Embassy of Malaysia will not accept registration of child born before 1^st January 2010. The mother of the child is advised to contact the National Registration Office (JPN) for further info. Please check on the website www.jpn.gov.my

For your information, it may take about one (1) year to receive an approval from the National Registration Office, Malaysia.

For successful application, it is compulsory that the mother and the child be present personally at the Embassy/Honorary Consulate to collect the Citizenship Certificate for the child. The Citizenship Certificate has to be collected within a year from the date of approval otherwise it will be returned to the National Registration Office. A fee of RM100.00 or equivalent in cash and acknowledgement receipt form C.15(2) are required. (form is available in Embassy)

Application for ´Mykid` Card must be obtained within five (5) years after receiving the Citizenship Certificate. Application must be done personally at the National Registration Office (JPN) in Malaysia.
