APPLICATION FOR CERTIFICATE OF WAIVER OF TWO YEAR HOME RESIDENCE (ONLY FOR MALAYSIAN)

 

 

NOTE:

  1. Applicant must be present at the Consulate General to submit the application. Kindly email to nycg.cons@kln.gov.my to set up an appointment.

  2. All applications will be forwarded to the Ministry of Foreign Affairs in Putrajaya, Malaysia for processing. The approximate time for processing and approval could be at least six (6) months. The application has to be referred to the relevant authorities in Malaysia for necessary clearance.

  3. Once the application has been approved, it will be forwarded to the Embassy of Malaysia in Washington D.C. and then will be submitted to the US Department of State.

  4. Only applicable to those who resides in the 17 States under the jurisdiction of the Consulate General of Malaysia, New York; Arkansas, Connecticut, Delaware, Iowa, Maine, Massachusetts, Michigan, Minnesota, Missouri, New Hampshire, New Jersey, New York, Oklahoma, Pennsylvania, Rhode Island, Vermont and Wisconsin.

1.

Three (3) copies of complete application form (Form is available at the Consular section of the Consulate General of Malaysia, New York)

 

2.

Original and three (3) copies of valid Malaysian passport (pages that contain details and signature of the applicant). The passport must have minimum validity of nine months

 

3.

Original and three (3) copies of Malaysian Identification Card – MyKad (front and back)

 

4.

Three (3) current photos (passport size)

 

 

5.

Third Party Barcode page issued by the U.S. Department of State.  The applicant must submit their application online:  https://j1visawaiverrecommendation.state.gov/

 

 

6.

Completed Contact Information Sheet (Available at the Consulate General of Malaysia, New York)

 

 

7.

Return postage by Express Mail (the Consulate General of Malaysia shall not be held responsible for any documents lost in transit)

 

8.

Fee of USD7.00 in cash or money order made payable to Consulate General of Malaysia. (Money order is accepted ONLY FROM The United States Postal Service, Western Union or MoneyGram)
