RE : APPLICATION FOR NEW/REPLACEMENT MALAYSIA PASSPORT (MRP)
Kindly submit the following documents:-
1. Application form IM.42
2. 1 photocopy of Birth Certificate (not applicable if you have MyKad)
3. 1 photocopy of Identity Card (back & front)
4. 1 photocopy of Citizenship Certificate (if applicable)
5. 1 photocopy of Passport (page 1, 2, 3 & Australian Visa )**
6. 2 recent passport sized photos (white background)
7. Children (Under 18) please include:
i. 1 photocopy of their Birth Certificate
ii. 1 photocopy of Parents’ Identity Card (back & front)
iii. 1 photocopy of Parents’ Passport (page 1, 2, 3 & Australian Visa )
iv. 1 photocopy of Parents’ Malaysian Marriage Certificate
8. Fee of AUD __________ cash
* Emergency Certificate: 1 photocopy of air ticket / itinerary

§ Do not cut photocopies of documents to size (must be A4 size)
§ Bring along your original documents when submitting your application at our office.
§ **Passport expired more than 6 months need to get “Certificate of Evidence of Resident Status in Australia”. (This certificate can be obtained from the Department of Immigration and Citizenship) OR Visa Entitlement Verification Online (VEVO)
