APPLICATION FOR LETTER VERIFYING MARITAL STATUS

This application is intended for verification of marital status of non-Muslims Malaysian citizens who wish to marry abroad.

For Malaysian applicants who were abroad, applications can be made through Consulate General of Malaysia:

• Applicant present at the Consulate General of Malaysia to fill up the JPN KC09 form;

• JPN KC09 form and all photocopies of required documents will be certified by the Consular Officer at the Consulate General of Malaysia office;

• The applicant must make a Statutory Declaration related to the latest marital status examples single / never married / divorcee at the Consulate office;

• Make payment at the Consulate General office

• Submit the JPN KC09 form and enclose all supporting documents (ID, passport or relevant documents) that have been translated (if applicable) and certified by the Malaysian Representative office and post to the address below:

(Documents sent by post must include the correct contactable correspondence address)

Required Documents:

✓ Form JPN.KC09 must be fully completed, particularly the ID number, names of spouses, date and place of marriage.

✓ MyKad (photocopy of MyKad if application is by post).

✓ Photocopy of passport pages with personal details of prospective spouse who is a non-citizen.

✓ Application letter stating the purpose of the application. (ORIGINAL)

✓ Statutory declaration stating the applicant’s current marital status. If the applicant is outside the country, then the declaration needs to be made before a Malaysian representative in that country. (ORIGINAL)

✓ Receipt of payment 

• The Letter of Confirmation of Marital Status will be posted directly to the applicant who is residing abroad or the Malaysian Representative Office in the country concerned or handed over to the next of kin authorized by the applicant.

• Fee (cash only)

✓ RM5.00 (to be paid when submitting the application at NRD).

✓ CNY44.00 fee for Statutory Declaration

✓ CNY10.00 each for certified true copies documents
