| i. | 2 copies of Application Form of Borang K JPN (Perkara 23-Kaedah 36) | |
| ii. | Malaysian Passport (original and 2 photocopies of all pages where something is stamped or written on, including German Residence Permit) | |
| iii. | Malaysian Identity Card (original and 2 photocopies) | |
| iv. | Malaysian Birth Certificate (original and 2 photocopies) | |
| v. | Proof of Malaysian Citizenship (if any) | |
| vi. | 2 Coloured Photographs (passport size) | |
| vii. | Letter of German "Einbürgerungszusicherung" with English translation by a certified translator (original and 2 photocopies) | |
| viii. | Fee: RM 10.00. It is payable in the Euro equivalent. | |
