| i. | Police report with English translation by a certified translator |
| ii. | Malaysian Birth Certificate (original and 2 photocopies) |
| iii. | Malaysian Identity Card (original and 2 photocopies) |
| iv. | Proof of work permit-only applicable if holder of lost passport is working in Germany (original and 2 photocopies) |
| v. | Letter from the employer in English- only applicable if holder of lost passport is working in Germany (original and 2 photocopies) |
| vi. | Proof of residence permit in English (original and 2 photocopies) |
| vii. | 4 recent coloured photographs, size 3.5.x 5.0 cm |
| viii. | Copy of lost passport (if available) |
| ix. | Letter from University in English( applicable only if holder of lost passport is a student)) |
