CONFIRMATION OF DEATH (MALAYSIAN CITIZEN)
The Consulate General of Malaysia, Chennai provides the service for confirming the death of her citizen in India and one has to provide the following:-
1. | Passport | Original & 2 copies |
2. | Malaysian Identity Card (If any) | Original & 2 copies |
3. | Other Documents | Original & 2 copies |
4. | Police Report | Original & 2 copies |
5. | Customs Declaration | Original & 2 copies |
6. | Attending Doctors Report On The Probable Cause Of Death or Autopsy Report | Original & 2 copies |
7. | Embalming Certificate or Cremation Certificate | Original & 2 copies |
Note:
- Malaysian Passport and Identity Card will be retained by the Consulate and a certified copy will be returned.
- Item 5, 6 & 7 is only required if the deceased needs to be transported back to Malaysia (Process Time: Immediately)
Process Time: Three (3) months from the date of submission.
