LETTER OF UNDERTAKING AND INDEMNITY FOR PERSON UNDERSURVEILLANCE (LOU)

All passengers travelling to Malaysia starting 24 July 2020 must; -

(kindly refer to NADMA Malaysia)

 

i. Complete and sign a Letter of Undertaking and Indemnity for Person Undersurveillance (LoU). All passengers may download the necessary documents and complete the LoU through the following links:

ii. Obtain a Letter of Approval from an accredited Malaysian Missions Abroad

• The entry requirement is in compliance with the terms set by The Malaysian Immigration Department or government agencies. Each applicant must submit the relevant documents along with the completed LoU to an accredited Malaysian Missions at least three days before departure date. 

• The email address of the Malaysian Consulate abroad can be found at https://www.kln.gov.my/web/guest/malaysianmission. The Malaysian Missions will issue Travel Notice (email submission), subject to approval. 

 

iii. Submission of the Travel Notice during the check-in process and the Malaysian Immigration Department 

• A letter of approval from the Malaysian Missions must be presented during the check-in process. Failure to submit the document for foreign travellers may result in being denied boarding.  However, Malaysian who are unable to submit the LoU prior departure, will still be allowed for boarding however required to complete the LoU upon arrival in Malaysia.

• Malaysians and foreigners returning to Malaysia are required to download the MySejahtera application on their phones. The app is developed by the Government of Malaysia specially for Malaysians and Non-Malaysians returning or visiting Malaysia starting from departure, upon arrival and during the 14-day quarantine. It is based on the standard operating procedures developed by the Malaysian government to manage the COVID-19 pandemic. The app may be downloaded via https://mysejahtera.malaysia.gov.my

 

b. Upon arrival 

The Person Under Surveillance (PUS) will undergo the following process: -

 

i. Health screening upon arrival will be conducted by the health authorities from Ministry of Health Malaysia (MOH)

•  Health screening will be conducted and any symptomatic passengers will be referred to designated hospitals for further treatment.

• The sampling process for the COVID-19 test will take place at the arrival hall, or at the quarantine station. The cost for COVID-19 test is as determined by MOH.

• Health Assessment Tool (HAT) will be provided for the purpose of self-monitoring of PUS throughout the quarantine period.

 

ii. Registration 

•  The Management Committee from the National Disaster Management Agency (NADMA) and the Malaysia Civil Defence Force (APM) will coordinate the reception of PUS and logistics management at KLIA1, KLIA2 or any other entry points via air, sea and land all over Malaysia.

• The PUS must present the LoU to the officer on duty.

• The Management Committee will manage logistics arrangement for PUS to the designated quarantine centres.

• PUS is not allowed to pick the quarantine centres.

 

iii. Immigration, Customs and logistics reception to the Hotel

• PUS will go through the usual entry procedures through Immigration and Customs Officers.

• The government will arrange transportation to designated quarantine centres.

• For foreigners who fail to submit LoU upon arrival, the Immigration has the right to order a Not-To-Land (NTL) directive.

Passengers are reminded to be aware of and check the travel rules and restrictions with respective embassies for your departure and arrival country to avoid from being denied permission to proceed for the flight.
