REGISTRATION OF CIVIL MARRIAGE

(THE EMBASSY DO NOT PROCESS ISLAMIC MARRIAGE REGISTRATION)

CHECKLIST

1. Form JPN.KC06 – Original (to be obtained from Embassy) & print-out one

2. Original & photocopy two (2)

     - Lebanese govt. marriage cert., issued by Births, Deaths & Marriages Registration Office

     - MyKad (all sides)

     - National ID card (for foreign citizen, if any)

     - Passport

     - Birth certificate

     - Travel document / visa

3. Applicants’ Passport size photo – Two (2)

4. USD 8.00 fee, cash only

5. Application time: 9.30 a.m. – 12.00 p.m. Monday to Friday.

6. Penalty fee applies if marriage is not registered within six (6) months

        7. Both husband and wife need to be present during registration

        8. Both husband and wife need to be present during collection of certificate

9. Processing time: Three working (3) days  

REGISTRATION OF BIRTH “BORANG D”

* THE FATHER IS A MALAYSIAN CITIZEN &

* THE CHILD IS BELOW ONE (1) YEAR OLD

CHECKLIST

1. Form JPN.WN02 – Original (to be obtained from the Embassy) & print-out one (1)

2. Original & photocopy two (2):

     - Child’s birth certificate

     - Child’s passport (if any)

     - Parents’ Malaysian marriage certificate

     - Parents’ MyKad (all sides)

     - Parents’ national ID card (for foreign citizen, if any)

     - Parents’ passport

     - Parents’ birth certificate

     - Parents’ travel document / visa

3. Child’s passport size photo – Two (2)

4. USD 8.00 fee, cash only

5. Application time: 9.30 a.m. – 12.00 p. Monday to Friday.

6. One (1) of the parents and the child needs to be present during registration

7. Processing time: Three (3) working days

* THE FATHER IS A MALAYSIAN CITIZEN &

* THE CHILD IS ABOVE ONE (1) YEAR OLD

CHECKLIST

1. Form JPN.WN03 – Original (to be obtained from the Embassy) & print-out one (1)

2. Attachment A.15(2) – Print-out on a single A4 sheet two (2)

3. Attachment A-WN – Print-out on A4 sheet two (2)

4. Original & photocopy two (2):

     - Child’s birth certificate

     - Child’s passport (if any)

     - Parents’ Malaysian marriage certificate

     - Parents’ MyKad (all sides)

     - Parents’ national ID card (for foreign citizen, if any)

     - Parents’ passport

     - Parents’ birth certificate

     - Parents’ travel document / visa

5. Child’s passport size photo – Four (4)

6. USD 8.00 application fee & USD 5.00 for issuance of the certificate

7. Application time: 9.30 a.m. – 12.00 p.m. Monday to Friday.

        8. Both parents and the child need to be present during registration

* THE FATHER IS NOT A MALAYSIAN CITIZEN &

* THE CHILD IS BELOW ONE (1) YEAR OLD

1. Form JPN.WN01B – Original (to be obtained from the Embassy) & print-out one (1)

2. Attachment A.15(2) – Print-out on a single A4 sheet two (2)

3. Attachment A-WN – Print-out on A4 sheet two (2)

4. Original & photocopy two (2):

     - Child’s birth certificate

     - Child’s passport (if any)

     - Parents’ Malaysian marriage certificate

     - Parents’ MyKad (all sides)

     - Parents’ national ID card (for foreign citizen, if any)

     - Parents’ passport

     - Parents’ birth certificate

     - Parents’ travel document / visa

     - Child’s siblings birth certificate & “Borang W” (if any)

5. Child’s passport size photo – Four (4)

6. USD 8.00 fee, cash only

7. Application time: 9.30 a.m. – 12.00 p.m. Monday to Friday.
