PASSPORT APPLICATION REQUIREMENTS

Effective 1 March 2009 all applications for Malaysian international passports will be forwarded to the Immigration Department of Malaysia, for processing and approval. The estimated time of process is between 1-3 months.

To ensure a prompt processing please make sure that all supporting documents are certified by a Notary Public. Failure to do so will only delay the application.

The passport is an important document, therefore it must be in the safe custody of the holder at all times.

All applicants MUST submit and collect documents personally at the Embassy.

Documents required are as follows:

· A completed Application Form IM.42

Forms may be downloaded from www.imi.gov.my (to be printed on both sides of paper) or collected from the Embassy.

· Two (2) Copies of Malaysian Identity Card

· A copy of old passport

· Thumbprint (left or right)

· Two (2) copies of Citizenship Certificate (if applicable)

· Two (2) copies of letter from your employer/learning institution confirming your status in Mexico

· Two (2) copies of proof of visa status (if your visa is attached in your old passport, kindly provide a certified copy)

· Two (2) recent passport-sized photos

For applications of children/infants, additional documents required are:

· A copy of father’s passport

· Form W/Birth Certificate

· Forms A1/B1 (for children below 12 years)

· Two (2) copies of Marriage Certificate (for first time application)

The fee must be paid in Mexican Pesos or US Dollar equivalent to RM300.00 according the the Government of Malaysia rate of exchange for a 48-page passport . Children below the age of 12 years old will be charged half of the fees.

The Embassy of Malaysia shall not be responsible for passports/documents lost in mail.

ISSUANCE OF NEW MALAYSIAN PASSPORTS IN THE CASE OF DAMAGED MALAYSIAN PASSPORTS

Documents required are as follows:

· Documents as in applying a new Malaysian passport

Additional documents:

· Damaged Passports Inquiries form (may be obtained from the Embassy…)

· A cover letter explaining why the passport is damaged

· Two (2) recent passport-sized photographs .

· fee must be paid in Mexican Pesos or US Dollar equivalent to RM300.00 for a 48-page passport.

ISSUANCE OF NEW MALAYSIAN PASSPORTS IN THE CASE OF LOST MAALYSIAN PASSPORTS

Documents required are as follows:

· Documents as in applying a new Malaysian passport

Additional documents:

· Loss Passports Inquiries form (may be obtained from the Embassy …)

· A cover letter to inform the details of the loss

· A copy of the local police report

· Two (2) recent passport-sized photographs

· Application form for new passport IM 47

The fee must be paid in Mexican Pesos or US Dollar equivalent to RM300.00 for a 48-pages passport.

REGISTRATION OF CHILDREN BORN IN MEXICO

Children born in Mexico can be registered at Embassy of Malaysia, Mexico City. Registration must be made in person at the Embassy on the following conditions:

1. The father of the child is a Malaysian citizen;

2. Registration is made within one (1) year from the date of birth of the child;

Documents required are as follows:

· Completed Birth Registration Form D (may be obtained from the Embassy)

· Passports of both parents (original and two (2) copies)

· Identification cards of both parents (original and two (2) copies)

(If the mother is not a Malaysian citizen then her passport will be used as an identity document)

· A copy of Marriage Certificate from Malaysia or other countries

· Translation of Mexican birth certificate

· Mexican Birth Certificate (for child)

· Two (2) passport-sized photographs of the child

The fee must be paid in Mexican Pesos or US Dollar equivalent to

MXP 110.00 (RM25.00)

Note: Borang W (Malaysian Birth Certificate Overseas) will be issued by the Embassy. A copy will be sent to the National Registration Department of Malaysia for record purposes.

**Penalty charges will be imposed for late registration

APPLICATION FOR CITIZENSHIP STATUS FOR CHILDREN BORN TO A NON MALAYSIAN FATHER (PERKARA 15(2) PERLEMBAGAAN MALAYSIA)

Applications for Malaysian citizenship will be forwarded to Jabatan Pendaftaran Negara at Putrajaya by the Embassy. Applications must be made in person at the Embassy on the following conditions:

1. Mother should be Malaysian

2. Child born after official marriage registration /soleminization

3. Child born overseas after 1 January 2010 or within one year after the child was born.

Documents required are as follows:

· Lampiran A.15(2) Surat Persetujuan Bersama Permohonan Kewarganegaraan Perkara 15(2) Perlembagaan Persekutuan

· Lampiran B.15(2) Akuan Penerimaan

· Lampiran C.15(2) Akuan Kutipan

· Two copies Birth Certificate of Child

· Two copies Father’s identity card of travel document

· Two copies Mother’s birth certificate or identity card

· Two copies Mother’s citizenship certificate (if applicable)

· Two copies Marriage certificate (translated in English of Bahasa Malaysia)

· Three (3) recent passport-sized photographs

The fee must be paid in Mexican Pesos or US Dollar equivalent to RM 1,000.00.

REGISTRATION OF MARRIAGE

Under the Marriage and Divorce Act 1976 of Malaysia, Malaysians residing overseas are required to register their marriage at any Malaysian mission for your marriage to be legally recognized in Malaysia. The married couple is required to appear in person when submitting the application form on the following conditions:

1. At least one of the couple is a Malaysian.

2. Register the marriage with the Embassy within the period of six (6) months from the date of marriage.

Documents required are as follows:

· Malaysian passports

· Malaysian Identity Card (MyKad)

· Mexican Marriage Certificate (and Divorce Certificate if applicable)

· Two (2) recent passport size photographs

The fee must be paid in Mexican Pesos or US Dollar equivalent to RM10.00.

**Penalty charges will be imposed for late registration

Registration will be processed by the Embassy and two (2) copies of the Malaysian Marriage Certificate (original and a photocopy copy) will be issued to the couple.

*NOTE: All marriages performed in the Islamic Religious Rights need not be registered with Embassy of Malaysia. Marriage registration can be done in Malaysia at any Registration Department, but all Marriage Certificates issued in Mexico must first be certified by the Embassy. Marriage Certificates from another country must be certified by the respective country’s Embassy, Consulate or Ministry of Foreign Affairs.

REPORTS OF DEATH OF MALAYSIANS OVERSEAS

The relatives or next of kin in Malaysia should report the death of the person to the Embassy of Malaysia. Information and documents required are as follows:

1. Name of the deceased

2. Passport and Identification Card Number

3. Address of the deceased abroad

4. Address in Malaysia

5. Address of the employer if any

6. Cause of death

The relatives or next of kin of the deceased will need to decide the place of burial, either abroad or in Malaysia.

Burial in Malaysia

· The Embassy will obtain the estimated cost of the repatriation which will have to be deposited by the relatives or next of kin to the Consular Division at Ministry of Foreign Affairs of Malaysia in Putrajaya. A receipt will be issued.

· The Embassy will carry out the repatriation process.

· The Consular Division will notify the family on the flight details for the arrival of the remains.

Burial Abroad

· The Embassy will assist in coordinating the burial process.

· All cost incurred will have to be borne by the relative or next of kin which have to be paid to the Consular Division at the Ministry of Foreign Affairs of Malaysia in Putrajaya. A receipt will be issued.

· The Consular Division will submit the following documents to the relative or next of kin:

- Acknowledgment letter from the respective mission

- Certificate of Health

- Certificate of Death

- Certificate of Burial

- Death Report

- Copy of Passport/Identification Card (original document will be submitted directly to the respective departments in Malaysia)

ARREST BY LOCAL AUTHORITIES

The relatives/family members/acquaintance may make a written report about the arrest/detained/deported person at the Consular Division, Ministry of Foreign Affairs at the following address:

The information required for the report are as follows:

· Name of the arrested/detained/deported person

· Passport and Identification Card Number

· Date and place of arrest/detention/deportation

· Telephone number of place of arrest/detention/deportation

· Cause of arrest/detention/deportation

The Consular Division and the respective Embassy shall carry out the investigation and the family will be notified of result of the investigation.

If the family requires legal assistance or need to visit the arrested/detained person, the application needs to be submitted to the Consular Division. All costs incurred will be borne by the family.

After completion of sentence (if found guilty), the person will be deported back to Malaysia.

In some countries, cost of deportation will be borne by the country concerned. In others, the family needs to bear the cost:

· Deposit the required amount to the Consular Division

· The family will be informed of the details of arrival

· The family will receive the person at the airport in Malaysia

ISSUANCE OF EMERGENCY TRAVEL DOCUMENT (EC)

The Emergency Travel Document (EC) is issued to Malaysians holding an expired, lost passport or passport validity less than 6 months to return to Malaysia one way via all countries except listed below.

Applications must be submitted one (1) month before departure. Documents required are as follows:

· A completed Application Form IM.42

Additional Documents

· Cover letter explaining why you need the Emergency Certificate

· Current Malaysian Passport (if applicable)

· Two (2) Notarized Copies of Malaysian Identity Card

· Two (2) Notarized Copies of Birth Certificate

· Two (2) Copies of Marriage Certificate (if applicable)

· Two (2) Copies of Permanent Resident Card

· Two (2) Copies of letter from your employer/school confirming your status

· Copy of Flight Details or Airplane Tickets

· Four (4) recent passport-sized photos

· For return postage or courier company account

The fee must be paid in Mexican Pesos or US Dollar equivalent to RM50.00

CERTIFICATE OF GOOD CONDUCT

Malaysians and foreigners (those who have stayed in Malaysia for 1 year and beyond) who wish to apply for the Good Conduct Certificate can do so at the Embassy of Malaysia,or apply online through the Ministry of Foreign Affairs website. However the original application should be forwarded to the Embassy to be forwarded to the Ministry of Foreign Affairs of Malaysia.

Below are the requirements for the application of Good Conduct Certificate:

For Malaysian

Three copies of the application from and online registration (e-CONSULAR)
*E-CONSULAR must be completed, printed and submitted together along with the 3 application form.

The Document require as follows:

1. Three (3) recent Passport size photographs (front view).

2. Three (3) certified copies of your Malaysian Identity Card.

3. Three (3) certified copies of your Passport namely pages that contain details of the applicant (front page and last pages).

4. Fees in Mexican Pesos,US Dollar equivalent to RM20.00

5.For your information, your application has to be referred to the relevant authorities in Malaysia for necessary clearance. You will be notified by mail once the Consulate receives the approval from Malaysia. Processing time will be within 4 to 6 months from the date of submission.

For Foreigner

Three copies of the application from and online registration (e-CONSULAR)
*E-CONSULAR must be completed, printed and submitted together along with the 3 application form.

The document require as follows:

1.Three (3) recent Passport size photographs (front view).

2.Three (3) certified copies of the Identity Card.

3.Three (3) certified copies of your Passport namely pages that contain details of the applicant (front page and last pages).

4.Fees in Mexican Pesos,US Dollar equivalent to RM20.00

RENUNCIATION OF CITIZENSHIP

All forms must be requested at the Embassy

Documents required are as follows:

1. Your original Malaysian International Passport

2. Your original Identity Card

3. Your original Malaysian Birth Certificate (The original Birth Certificate will be returned to you once your renunciation is completed)

4. Two (2) recent passport size photographs.

5. Copy of your current nationality passport and Naturalization Certificate.

6. Fee of RM12.00 or equivalent to MXP or USD.

7. If you have lost your original Malaysian Identity Card, Malaysian Birth Certificate or both, please lodge a Lost Property Report from your local Police Department and send a copy of your report to this office for processing.

*NOTE: Your application will be sent to Jabatan Pendaftaran Negara in Putrajaya, Malaysia. The processing will take between 4 to 6 months from the date of submission to this office.
