v. DSWD Clearance for minor applicants travelling with guardian

vi. One (1) copy sponsor’s latest bank account statement.(3 months)

vii. One (1) copy of round trip ticket

viii.One (1) copy of applicant's passport information page

ix. One (1) piece of passport size photo of the applicant (white background)

ii. Original and copy Alien Certificate of Registration Card(ACR-Card)

iii. Hotel reservation/ Notarized Affidavit of Invitation verified by Ministry of Foreign Affairs with Sponsor’s

iii. Notarized Employment Contract. (for Domestic Helper, contract must be authenticated by Embassy of Philippines in

Kuala Lumpur)

v. One (1) piece of passport size photo of the applicant (white background)

DEPENDENT VISA

ii. Original MDeC/Immigration Malaysia Approval Letter

iii. One (1) copy of main sponsor’s passport and active visa

iv. Primary sponsor’s copy of employment contract

vi. Proof of relationship to the main sponsor (NSO issued marriage or birth certificate)

Schedule of Fees

Note: -

-Passport must be valid for at least 6 months

-Nigerian and African nationals are required to submit yellow fever vaccination card

Note: Visa is not required for a stay of less than one (1) month for ASEAN nationals except Myanmar. Visas are required for duration of stay that exceeds one (1) month except for Brunei and Singapore nationals.
