The applicant must submit the following documents:
1) A Copy of birth Certificate (Malaysian birth certificate)
2) A Copy of Parents IC or ID card
3) A Copy of Parents passport
4) Three (3) passport size photographs of the underage (with white background)
5) Flight itinerary (if have)**For babies, the applicant must bring the baby together on the submission date for the finger stamp on the form. The process will take three (3) working days.
