Emergency Certificate (Lost /Damage/ etc.)

The applicant must submit the following documents:

1. Police Report (Original in Thai and translated to English)
2. A Copy of IC 
3. A Copy of Passport 
4. Flight itinerary (if have) 
5. Applicant to complete the questionnaire form

**The applicant will be interviewed by the consular officer prior to the issuance of the emergency certificate.

** The process will take three (3) working days.
