Birth/Marriage Registration

LIST OF DOCUMENTS REQUIRED

FOR THE APPLICATION OF A MALAYSIAN BIRTH CERTIFICATE

AT THE EMBASSY OF MALAYSIA IN ASHGABAT, TURKMENISTAN

1. A local birth certificate issued in the host country to be translated into English (please make sure that the name of a child and a father and his nationality are written correctly) and certified true copy.
2. Application form [Borang Pemohonan Daftar Lahir Luar Negara – Borang D (JPN.WN02)] to be filled and three (3) 3 cm x 4 cm passport size photos;
3. Photocopies of the parents’ passports with valid visa;
4. Photocopies of IC/ID of parent(s);
5. A photocopy of the Malaysian marriage certificate;  
6. A photocopy of the mother’s birth certificate; and
7. Consular Fee – USD 30.00 in cash.

Note: 

Application and issuance of the Malaysian Birth Certificate must be done not later than one year from the date of birth. Both birth parents are to be present during registration at the Embassy.

(Note: All original documents must be presented at the Embassy for verification)

FOR THE APPLICATION OF A MALAYSIAN MARRIAGE REGISTRATION

AT THE EMBASSY OF MALAYSIA IN ASHGABAT, TURKMENISTAN

1. Application form (JPN.KC 06);
2. Photocopies of Marriage certificate;
3. Photocopies of couple’s IC/ID;
4. Photocopies of couple’s birth certificates; and
5. Photocopies of recent passport-size photos (3 cm x 4 cm).
6. Please submit three (3) copies of the application forms containing item 1-5.
7. Consular fee – USD 10.00 in cash.

Note:

            Applicants to fill the form in capital letters, using only black/blue ballpoint pen.
