REQUIREMENTS FOR PASSPORT APPLICATION

 

  1. ADULT
  1. Passport application form (IM.42-PIN.1/97). Application form is available at the Embassy;
  2. Two (2) photocopies of Malaysian identity card (IC);
  3. Two (2) photocopies of old passport (Biodata page);
  4. Three (3) passport size photographs (white background) 3.5cm X 5.0cm;
  5. Two (2) photocopies of valid working visa/residence permit;
  6. Support letter from company / employer;
  7. Passport fee: RM 200.00,
    (in USD according to Government of Malaysia’s monthly exchange rate);
  8. Processing time is within 5 months.

 

  1. PASSPORT APPLICATION FOR A NEW BORN CHILD
  1. Passport application form (IM.42-PIN.1/97). Application form is available at the Embassy;
  2. Two (2) photocopies of registration certificate of the child (Borang H);
  3. Two (2) photocopies of parents/guardians passport;
  4. Two (2) photocopies of parents/guardians identity card (IC);
  5. Two (2) photocopies of parents/guardians marriage certificate;
  6. Two (2) photocopies of parents/guardians valid working visa/residence permit;
  7. Three (3) passport size photographs (white background) 3.5cm X 5.0cm;
  8. Passport fee: RM 100.00,
    (in USD according to Government of Malaysia’s monthly exchange rate);
  9. Processing time is within 5 months.
