INSTRUCTIONS FOR EMERGENCY PASSPORT AT THE EMBASSY OF MALAYSIA, WASHINGTON D.C.

 

Emergency Certificate is used for the purpose of travelling back to Malaysia (one way) due to an emergency/lost passport/expired passport. Once in Malaysia, you are advised to contact Immigration Department in Malaysia to obtain a new passport for all of your future travel plans. In addition, please plan your trip accordingly as the following countries do not recognize our Emergency Certificate in lieu of a passport.

           

Brunei • Canada • Chile • France • Germany • Holland • Iran • Iraq • Italy • Japan • Jordan • Kuwait • Norway • Oman • Papua New Guinea • U.A.E • Senegal • Sudan • Switzerland • Turkey • China

 

You are required to obtain the following;

  • A written letter requesting the need for an Emergency Certificate
  • Original Passport
  • Fully completed, dated and signed IM42 Form (please sign section E. 13(d) and NOT section F. 14)
  • Two (2) notarized (clear original copies) of your Malaysian Identity Card (front & back)
  • Two (2) notarized (clear original copies) of your Malaysian Birth Certificate
  • Two (2) notarized (clear original copies) of your Malaysian Birth Certificate (if applicable)
  • Fully completed Registration of Malaysian Citizens in the USA form
  • Contact information form
  • A copy of your flight itinerary OR copy of airline ticket
  • Fee of USD$16.00 payable to “Embassy of Malaysia” by USPS money order only (available from your local Post Office). Personal checks are NOT accepted.
  • Three (3) passport-sized photograph – 2inch x 2 inch / white background only; taken within 2 weeks (please do not attach to application form)

 

IMPORTANT:

  • Emergency Certificate (Travel Document) has limited validity from the date of issue.  Kindly plan your trip accordingly;
  • And it can only be used ONE TIME
