· Cover Letter explaining why the Passport is damaged
· 2 Copies of Malaysian Identity Card
· 2 Copies of Birth Certificate
· 2 Copies of Citizenship (if applicable)
· 2 Copies of Marriage Certificate (if applicable)
· 2 Copies of Permanent Resident Card
· 2 Copies of letter from your employer/school confirming your status
· 2 Copies proof of visa status
· The fee via US Postal Money Order (NO PERSONAL CHECK) made
payable to Embassy of Malaysia USD80.00 = MY 300.00(for 32
pages)
· Four (4) passport-sized taken within 2 weeks (Foto requirement )
· For return postage US$17.50 postage stamps- (one single stamp).
IMPORTANT NOTE:
- Please be sure to place your right thumb print the application form IM.4 in black ink
- All the applications will be send back to Malaysia and will be investigated by the Immigration Department in Malaysia for approval and issuance of new passport. This process will take between three to six months (sometimes more) to complete.
- All supporting documents MUST be certified by a Notary Public, failure to do so will only delay the process of your application
