Birth/Marriage Certificate Registration
LIST OF DOCUMENTS REQUIRED
FOR THE APPLICATION OF A MALAYSIAN BIRTH CERTIFICATE
AT THE EMBASSY OF MALAYSIA IN TASHKENT, UZBEKISTAN

A local birth certificate issued in the host country to be translated into English by a Notary office and certified at the Ministry of Justice and at the Ministry of Foreign Affairs (please make sure that the name of a child and a father and his nationality are written correctly. The Embassy recognizes only the English version spelling of above-mentioned details);

Application form (Borang H) to be filled and to paste one (1) 3 cm x 4 cm passport size photo on the form, a child's right and left thumbprints and father’s right thumbprint to be impressed on the form;

A photocopy of the local birth certificate and its certified copy;

Photocopies of the passports of parents;

Photocopy of IC/ID of parent(s);

A photocopy of the Malaysian marriage certificate;

A photocopy of a legal permit of stay in Republic of Uzbekistan for both parents;

A photocopy of the mother’s birth certificate; and
-
Consular Fee – USD12.00.
Note:
Application and issuance of the Malaysian Birth Certificate must be done not later than one year from the date of birth.
