 Lost of Malaysian International Passport

Lost of passport must be reported to the Embassy immediately. Documents to be submitted are :-

• Police report with English translation by a certified translator
• Malaysian Birth Certificate (original and 2 photocopies)
• Malaysian Identity Card (original and 2 photocopies)
• Letter from the employer in English (original and 2 photocopies)-only applicable if holder of the lost passport works there
• Letter from University in English (original and 2 photocopies)-only applicable if holder of the lost passport is a student
• 4 recent coloured photographs, size 3.5 x 5.0 cm
• Copy of the lost passport (if available)
