This document is issued to Malaysians holding an expired, lost passport or passport validity less than 6 months to return to Malaysia one way via all countries except listed below.

· Current Malaysian Passport (if applicable) |
· 2 Notarized Copies of Malaysian Identity Card |
· 2 Notarized Copies of Birth Certificate |
· 2 Copies of Marriage Certificate (if applicable) |
· 2 Copies of Permanent Resident Card · 2 Copies of letter from your employer/school confirming your status · Copy of Flight Details or Airplane Tickets · The fee MYR50.00 |
· Four (4) Recent passport-sized Photograph |
