APPLICATION FOR EMERGENCY CERTIFICATE

EMERGENCY CERTIFICATE / SIJIL PERAKUAN CEMAS

FOR NEW BORN IN CHINA
The father of the newborn must be a Malaysia Citizen to apply for the Borang H. Borang H is the primary document for issuing a Malaysian Emergency Certificate for the newborn in Mainland of China to perform the journey to Malaysia. You may apply for an Emergency Certificate once complete the process of Borang H.
Kindly make (01) photocopy of the following documents for the Malaysian Emergency Certificate application:

| 3. | Parents Passport and Identification Card
[biodata page, valid China Visa page (for Malaysian), observation page of the passport] |
| 4. | Marriage Certificate |
| 5. | Two (02) passport size photograph with white background.

| 6. | Fill up Emergency Certificate (EC) application form (IM.42) which can get over the counter in Visa Section. (Write the residence address in China in Pinyin). |

The applicant must bring all original and photocopied documents to verify by the officer in the Visa Section.
The validity of the Malaysian Emergency Certificate (EC) is three (03) months (authority of Immigration Attaché maximum up to 9 months) from the date of issuance. EC can use for Single Journey only from the Mainland of China to Malaysia. EC holders can’t transit in any other country.
Kindly refer to the National Immigration Administration (NIA) of China or China Embassy in Malaysia to clarify your China Visa matters. All the EC holders must get Exit Permit from the NIA of China before departure to Malaysia.
No prior appointment is required for EC application and applicant may come during operation hours as follows:
Monday-Friday : 09.00 a.m. – 02.30 p.m.
Lunch : 12.30 p.m – 02.00 p.m
Saturday, Sunday and Public Holiday : Closed

B. MALAYSIA CITIZEN LOST PASSPORT IN CHINA
Malaysian traveller/long term China Visa holder who lost their passport in Mainland of China must lodge a police report regarding their passport lost to a nearby police station. Police report should be translated by the local authority / Notary Public in English.
Applicant must complete the Borang Soal Selidik Kehilangan Passport Malaysia and meet Consular in Consular Section.
Consular will interview and issue an official letter/ Statutory Declaration to Visa Section to issue an Emergency Certificate for the applicant. The applicant must photocopy all the original documents from the Consular Section before submitting for Emergency Certificate.
Applicant must bring the following items for successful Emergency Certificate submission:
| 1. | Fill up Emergency Certificate (EC) application form (IM.42) which can get over the counter in Visa Section. (Write the residence address in China in Pinyin). |
| 2. | Two (02) passport size photograph with white background. Size specification as the attached photo: 35mm x 50mm

C. PASSPORT DAMAGE DURING THE STAY IN CHINA
Malaysian traveller/long term China Visa holder should apply for a Malaysian Emergency Certificate (EC) if their passport gets damaged during the stay in China.
Applicant must bring the following items for successful Emergency Certificate submission:
