The death has to be reported at:

Police report (original and 2 photocopies)

Malaysian passport of the deceased

Malaysian Identity Card of the deceased

Birth certificate / Marriage certificate family members of the deceased (documents that can prove the family relationship)

Death Certificate

Embalming Certificate
Note:

No. 5 and 6 need only to be submitted if the body is flown back to Malaysia.

Foreign documents issued in China must be notarized by a notary public followed by double authentication at the Foreign Affairs Office of Guangdong for documents issued in Guangdong. For other provinces, please refer to your Provincial Foreign Affairs Offices.
