Birth/Marriage Registration

CHECKLIST FOR

REGISTRATION OF BIRTH FOR MALAYSIANS 

1. Children born in Cuba, Bahamas, Dominican Republic, Haiti, Jamaica and Nicaragua can be registered at the Embassy of Malaysian, Havana on the conditions:

1. Father of the child is a Malaysian citizen; and
2. Registration is made WITHIN one year from the date of birth.

2.  The registration can be made by mail or physically in person at the Embassy of Malaysia, Havana, Cuba. 

Required documents are as follows:

3. 2 complete Forms JPN.WN02 (available at the Embassy of Malaysia):

1. Child’s Birth Certificate (Original & copy) notarised/endorsed by the Ministry of Foreign Affairs of the host country;
2. Translation of the Birth Certificate in English, notarised/endorsed by the Ministry of Foreign Affairs of the host country;
3. Malaysian Marriage Certificate for parents (Original & copy) ;
4. 3 pieces photos of the child (35 mm x 50 mm with blue background);
5. Payment: EUR 10.00 cash.

4. Documents (for Malaysian parents):

1. Identity Card (Original & copy) – front & back portion;
2. Birth Certificate (Original & copy); and
3. Malaysian International Passport (Original & copy).

5. Documents (for non – Malaysian mother):

1. Identity Card (Original & copy) notarised/endorsed by the applicant’s Embassy/the Ministry of Foreign Affairs of the host country;
2. Birth Certificate (Original & copy) notarised/endorsed by the applicant’s Embassy/the Ministry of Foreign Affairs of the host country; and
3. International Passport (Original & copy) notarised/endorsed by the applicant’s Embassy/the Ministry of Foreign Affairs of the host country.

Note:

• Both parents and child should be present at the Embassy of Malaysia in Havana, Cuba to process the birth registration.

CHECKLIST FOR

REGISTRATION OF MARRIAGE

A.     Application Documents:

1. 2 complete Forms JPN.KC06 (available at the Embassy of Malaysia);
2. Marriage Certificate (Original & copy) endorsed by the Ministry of Foreign Affairs of the host country;
3. 2 copies of the translation of the Marriage Certificate in English, endorsed by the Ministry of Foreign Affairs of the host country;
4. 2 pieces of latest photos from both parties (passport size : 35 mm x 50 mm with white background);
5. Payment: USD5.00 @ EUR 4.00 (cash).

B.     Documents for Malaysian applicant:

1. Malaysian Identity Card:  Original & 2 copies (front & back portion);
2. Malaysian passport.  Original & 2 copies (including visa page); and
3. Malaysian Birth Certificate.  Original & 2 copies.

C.     Documents for non – Malaysian applicant:

1. 2 copies of Birth Certificate endorsed by the applicant Embassy (if any) OR the Ministry of Foreign Affairs of the host country;
2. 2 copies of Local ID / Citizenship Certificate endorsed by the Ministry of Foreign Affairs of the host country; and
3. 2 copies of International Passport endorsed by the Ministry of Foreign Affairs of the host country.

Note:

• Both parties (bride & groom) should be present at the Embassy of Malaysia in Havana, Cuba to process the marriage registration.
• Registration of Marriage at the Embassy of Malaysia should be done WITHIN 6 MONTHS after the Wedding Ceremony taken place.
• All marriages performed in the Islamic Religious Rights do not need to register their marriage with Embassy of Malaysia. Marriage registration can be done in Malaysia at any Registration Department, but all Marriage Certificate issued in Cuba, The Bahamas, the Dominican Republic, Haiti, Jamaica and Nicaragua must first be certified by the Embassy. Marriage Certificate from another country must be certified by the respective host country’s Embassy, Consulate or the Foreign Ministry.  

• Please follow the aforementioned instructions carefully.
• The Embassy of Malaysia will not be responsible for passports/documents lost in mail.
