CHECKLIST FOR

MALAYSIAN INTERNATIONAL PASSPORT RENEWAL

 

  1. DOCUMENTS TO BE SUBMITTED TO THE MALAYSIAN EMBASSY:

 

  1. 2 completed Forms IM 42 Pin. 1/97 (with thumb print)  which can be downloaded here;
  2. 2 copies of the previous Passport;
  3. 2 copies of Malaysian Identity Card (front & back portion on the same page);
  4. 2 copies of Birth Certificate (if any) – for children below 12 years old;
  5. 2 copies of Parents’ Marriage Certificate (for applicant 18 years & below);
  6. 2 copies of Residence documents:
  • Permanent Residence (PR) card;
  • Valid Working Permit;
  • Valid Student Pass;
  • Dependent Pass, and
  • Other relevant documents.

 

  1. 4 pieces of latest passport size photograph (a sample is as attached);
  2. A self-addressed DHL envelope already prepaid in to facilitate the process of sending new passport back to applicant;
  3. Passport fees: EUR 50.00
  4. Passport fees:
  • Normal applicant (13 - 59 years old) : EUR 50.00/applicant;
  • Senior citizens (above 60 years old) : EUR 25.00/applicant;
  • Children (12 years and below) : EUR 25.00/applicant;
  • Students in the degree program studying abroad (below 21 years old): EUR 25.00;
  • Disabled (OKU): No fees applicable.

 

Payments could be made to the Embassy of Malaysia in Havana, Cuba bank account, and the details are as follows:

Note:

  1. Documents 2 – 7 shall be certified by the Notary Public beforehand.
  2. Documents in foreign languages should be translated into English or Bahasa Malaysia by a certified translator.
 
  1. Specifications of Photo for Malaysian International Passport
  • Applicants should wear dark-coloured clothing covering shoulders and chest (shirt with collar);
  • Taken in full-face view directly facing the camera with a neutral facial expression, both eyes open and mouth closed;
  • Do not wear a hat or head covering/obscures hair or hairline unless worn daily for a religious purpose.  Full face must be visible, and the head covering must not cast any shadows on your face;
  • Contact lenses and spectacles are not allowed while taking pictures, and
  • The applicants who wear veil (hijab) are required to wear dark-coloured veil.

 

Note:

  • All the above documents (once completed) have to be forwarded by
    courier to the Embassy of Malaysia in Havana, Cuba.
    Kindly be reminded that the Embassy of Malaysia will not be responsible for passports/documents lost in mail.

 

  • All applications will be forwarded to the Malaysian Immigration Department, Putrajaya, Malaysia for processing and approval.  The estimated time of process is between 3-6 months.

 

  • Please follow the aforementioned instructions carefully.
