Birth/Marriage Certificate

1. REGISTRATION OF MARRIAGE

Registration of marriage can be made in within six (6) months after your date of marriage in France or six (6) months upon arrival in Malaysia.  Both couples are to be present for the registration and the documents required are as followed:

  1. One form JPN.KC 06 to be completed at the Embassy. 
  2. Marriage Certificate (Copie Intégrale de l’Acte de Mariage) to be translated in English language by a qualified translator (www.cetiecap.com)
  3. Two (2) recent photograph (husband and wife).
  4. Original and copy of the passport (husband and wife).
  5. Copy of foreign husband or wife birth certificate to be translated to English language by a qualified translator.
  6. Copy of Malaysian Identity Card and Malaysian birth certificate, husband or / and wife, if applicable. 
  7. Fees of five (5) Euros by cheque payable to ‘Ambassade de Malaisie’.
  8. Marriage photograph at the City Hall (Mairie) with the Mayor.

Registration of marriage can be made after 6 months of delay, a sum penalty will be charged for the late registration.  

 

2. REGISTRATION OF BIRTH

Registration of birth has to be made in within one (1) year after the date of birth. The father and the child are to be present for the registration and the documents required are as follows:

  1. Registration of Birth Form (Borang D) to be completed at the Embassy.
  2. Birth Certificate issued by the French Authority (Copie Intégrale de l’Acte de Naissance) and to be translated to English language.
  3. 2 passport-sized photographs (Child).
  4. Copy of Malaysian/foreign passport of both parents.
  5. Copy of Malaysian/foreign identity card of both parents.
  6. Copy of Malaysian/foreign (translated birth certificate of both parents).
  7. Copy of resident card (Carte de Séjour) of both parents.
  8. Copy of Malaysian Marriage Certificate / Sijil Nikah. 
  9. Fees of 12 Euros. Payment by cheque payable to ‘Embassy of Malaysia’ or bank transfer.
