APPLICATION OF LETTER OF GOOD CONDUCT

Please submit application through e-consular

After completion, please send all supporting documents to the Embassy of Malaysia in Paris to be forwarded to Malaysia:

i)             Application of Malaysian Citizen

a)            2 copies of the duly completed application form for the Good Conduct

b)            2 certified copied of identification card

c)            2 certified copies of passport namely pages that contain details of signature of the applicant; and

d)            2 recent passport-sized photographs

e)            2 copy of permit stay(Carte de Séjour) in France/Portugal

f)             Fees of five(5) Euros

g)            Self-address enveloped with stamp

ii)            Application from non-citizen

a)    2 copies of the duly completed application form for the Certificate of Good Conduct

b)    1 introduction letter from the employer/Higher Learning Institution

c)    2 certified copies of passport namely pages that contain details, visa stamp and signature of the applicant; and

d)    2 recent passport-sized photographs

e)    Fees of five(5) Euros

f)     Self-address enveloped with stamp

3.  Application will be sent to Malaysia and process will take about 2 months and our address and opening hours are as below:

VISA APPLICATION TO MALAYSIA

The following documents are required for application of an entry visa :

a)Completed visa form

    website: http://www.imi.gov.my/images/Forms/visa/IM47.pdf

b)Passport or travel document with validity of minimum six(6)months.

c)Two(2) recent passport-sized photographs

d)Reservation of return flight air-ticket

e)Photocopy of permit stay(Carte de Séjour)  in France

f)Visa fees of five(5) Euros (except Chinese passports : Eight(8) Euros and Indian passports : Thirteen(13) Euros.
