Birth/Marriage Certificate
REGISTRATION OF BIRTH
Documentation required for the registration of birth / citizenship for child with a foreign father:
Child’s documents:
a) 3 photographs with blue background
b) RM 20.00 equivalent to € 5.00
c) Identity Card of child if any
d) Birth Certificate of child
e) Schooling certificate or letter from the school that child is attending
f) Passport or another document
g) Borang D
Parents documents:
a) Identity Card or death certificate
b) Birth Certificate or latest birth extract
c) Citizenship certificate
d) Passport
e) Stay Permit of foreign country
f) Marriage Certificate
g) Divorce certificate if any
h) If not married (appendices BWN/TK/01 or BWN/TK/02)
Sibling Documents:
a) Birth Certificate
Parents and the child must be present at the moment of registration.
For those submitting the application at the Malaysian Embassy’s it is acceptable to bring a foto with white background.
Documents in foreign language must be translated to English or Malay and authenticated by the Procura
(Public Prosecutor’s Office).
The copy of the documents birth/marriage/schooling abroad must be certified by the Embassy.
*Note
Muslim’s need to register their marriage at the Registration Department in Malaysia and if they have children they need to register them in Malaysia not with our Embassy.
Documentation for Registration of Birth for child with Malaysian father:
Registration should be submitted within 1 year of the child’s birth. For late registration they can apply directly in Malaysia.
a) Birth certificate (Plurilingue international certificate, if Italian Birth Certificate it needs to be translated and authenticated by the Procura)
b) 1 passport size photograph
c) Borang D
d) Document of Parents:
- Passport
- Identity card
- Birth certificate
- Marriage certificate
- Stay permit
e) Fee: EURO 7.00

Muslim’s need to register their marriage at the Registration Department in Malaysia and if they have children they need to register them in Malaysia not with our Embassy.
REGISTRATION OF MARRIAGE
For Malaysians that marry in Holy See and wish to register their marriage in Malaysia the following documentation is required:
a) One form JPN.KC 06 to be completed
b) Marriage Certificate to be translated in English language by a qualified translator
c) Two (2) recent photograph husband / wife
d) Passport and copy husband / wife
e) Copy of foreign husband / wife birth certificate to be translated to English language by a qualified translator
f) Malaysian Identity Card and copy
g) Fees: EURO 5.00
Registration of marriage can be made after 6 months of delay; a sum penalty will be charged for the late registration. Fine of RM 100.00 for the first year and RM 50.00 for each following year.
