     APPLICATION FOR A NEW/RENEWAL OF MALAYSIAN PASSPORT

    Checklist For Malaysian International Passport (PMA) Application Documents

 

    1.  One (1) copy of the completed Application Form (IM.42-Pin.1/97) – can be downloaded at

           https://www.imi.gov.my/portal2017/index.php/ms/muat-turun-borang-i.html / hard copy can

         be obtained at the Embassy of Malaysia.

 

    2. Original & 2 photo copies of:

 

       -  Malaysian International Passport (For copies, page 1 to 7 and pages have endorsement);

       -  Malaysian Identity card / Malaysian Birth Certificate / Citizenship card;

       -  Permanent Resident card;

       -  Employment letter from the company;

       -  Letter from university, college or school, confirming the applicant is a bona fide student

          together with the applicant study visa issued by the Cambodia       

          Immigration and Citizenship Department; and

       - Work visa issued by the Immigration and Citizenship Department.

 

   3. Three (3) recent, passport size photographs with White background. (Size 3.5 cm x 5.0 cm)

 

   4. Passport fees: USD60.00  (Payment in cash only)

 

  The Children’s passport under 18 years old

 

  1.  Additional documents for children's passport applications under 18 years old:

 

  • One (1) copy of Parents Consent Form Borang A1/B1 (IM.42A-Pin1/93 – can be downloaded at https://www.imi.gov.my/portal2017/index.php/ms/muat-turun-borang-i.html / hard copy can be obtained at the Embassy of Malaysia;
  • Birth Certificate/Mykid (original and two (2) photocopies) - for Malaysians under the age of 12 years and first time applicants;
  • Parents Passports (original and two (2) photocopies pages 1-7);
  • Parents Malaysian Identity Cards (original and two (2) photocopies);
  • Parents Permanent Residence Card (Malaysians only)/Work Permit/Study Permit (original and two (2) photocopies); and
  • Parents Marriage Certificate (original and two (2) photocopies) – for first time applicants.

 

  Note:

 

  All applicants must submit their applications in person. Application by mail will not be

  entertained.

 

  Frequently Asked Questions (FAQ)

 

   1. How long does it take to receive a new passport?

 

       Applicant will usually receive the new passport between three (3) to four (4) months from

       the Embassy depending on the approval by Passport Section, Immigration Malaysia

       Headquarters, Putrajaya.

 

   2. My Cambodia visa has expired, can I apply for the manual extension?

 

       No. Cambodia visa must be valid before applying for the manual extension.

 

   3. How much do I have to pay for a new passport?

 

        Fees are USD60.00 per person. For children under 12, and senior citizens above 60, the

        fees are USD30.00 per person

 

   4. What should I do if my passport is stolen or lost?

       

       You must lodge a police report at the nearest Cambodia Police station and provide a copy

       of the report to the Embassy of Malaysia, Phnom Penh.

 

   5. When can I start renewing my (PMA) passport?

 

       You can start renewing your passport as early as 12 months or a year before the passport 

       expires, to avoid delays in daily work and renew the Cambodian visa.

 

   6. My passport has expired, what should I do?

 

       First of all, an expired passport cannot be renewed at the Embassy and the applicant must 

       return to Malaysia to make a new passport (PMA);

 

       Second, the Applicant has to pay a fine/penalty at

       Immgaration Cambodia;

 

       Third, Bring a copy of the plane ticket and receipt of payment of fine/penalty from

        Immigartion Cambodia and apply for a temporary passport at the Malaysian

        Embassy to return back Malaysia.
