Birth Certificate Registration
i. A child born overseas to Malaysian parents or father (if mother is non-Malaysian) has to be registered at the Embassy within one (1) year from the date of birth;
ii. After the above said period the child can only be registered at the Malaysian Registration Department in Malaysia; and
iii. A Malaysian passport can only be applied for the child, after Form W has been issued by the Embassy.
Procedures
i. Birth certificate issued by the State of Kuwait has to be duly attested by the Ministry of Foreign Affairs of the State of Kuwait.
ii. Duly complete Form D is only available at the Embassy.
(Note: The parents/parent has to bring the child to the Embassy to complete procedures required in Form D)
iii. Upon completing Form D, Form W (Birth Certificate Registration) will be issued by the Embassy.
iv. Documents needed:
- One (1) copy of parents' passport (regardless if mother is non-Malaysian)
- One (1) copy of parents' Malaysian IC (only applies for Malaysian parents/parent)
- Four (4) copies of passport-sized photos of the child (close-up)
- One (1) copy of parents' marriage certificate
(Note: Please bring along the original set of the documents mentioned above for verification. If any of the originals are not available, then attested copies by the Ministry of Foreign Affairs of Malaysia is acceptable)
Fees
i. Form D: RM 20.00
ii. Form W: RM 10.00
