      - only with endorsement from the Ministry of Education of Malaysia and Consular Division of the Ministry of Foreign Affairs of Malaysia

REQUIREMENTS:

1. Original and 1 copy.
2. The fee is SAR 20.00 (Seen and Certified True Copy)

LETTER OF GOOD CONDUCT

(Approved Online Application from the Ministry of Foreign Affairs of Malaysia)

REQUIREMENTS:

1. Applicant's passport - to bring original and 2 copies.
2. Malaysian identity card (Iqamah identity card / passport for foreigner) - to bring original and 2 copies.
3. Print-out of transaction of online application.
4. Print-out of notification of approval from the online application.
5. The fee is SAR 20.00.

NOTARIAL SERVICES

TYPES OF DOCUMENTS:

1. Summon Suit from Malaysian State Court or High Court.
2. Documents notarised by the public notary in Saudi Arabia.
3. Sales and Purchase Agreement (S&P) from Malaysia.
4. Attestation of seal from the Ministry of Foreign Affairs of Malaysia and Saudi Arabia.

REQUIREMENTS:

1. Original documents and 1 copy.
2. Letter from the lawyer.
3. Identification document of the signee(s) and 1 copy.
4. The fee is SAR 20.00.

REPORT OF LOST MALAYSIAN PASSPORT

Please bring/submit the following:

1. Completed Borang Permohonan Pasport (Form IM.42);
2. Completed Borang Soal Selidik Kehilangan Pasport Malaysia (Form IM.42c)
3. Original and one copy of MyKad (front and back);
4. Original and one copy of Iqamah;
5. Original and one copy of a letter from your employer or school to confirm your present employment or studies;
6. Police report indicating the loss of your passport with English translation; and
7. Two (2) recent passport size photograph.

REPORT OF DAMAGED MALAYSIAN PASSPORT

Please bring/submit the following:

1. Completed Borang Permohonan Pasport (Form IM.42);
2. Completed Borang Soal Selidik Kehilangan Pasport Malaysia (Form IM.42c)
3. Original and one copy of MyKad (front and back);
4. Original and one copy of Iqamah;
5. Original letter from your employer or school to confirm your present employment or studies; and
6. Two (2) recent passport size photograph.

APPLICATION TO RENOUNCE MALAYSIAN CITIZENSHIP

REQUIREMENTS:

1. Malaysian Passport (original and 1 photocopy);
2. Malaysian Identity Card (original and 1 photocopy)
3. Birth Certificate or Citizenship (original and 1 photocopy);
4. Confirmation Letter of Application for Naturalisation as a Saudi Arabian (original and 1 photocopy) or original and copy of existing foreign passport;
5. Applicant has to fill in a Statement Form (2 sets); 
6. Applicant has to sign and Affix its thumbprints in "Borang K";
7. 3 pieces if recent passport-sized photos (blue background); and
8. The fee is SAR 20.00.

NOTE:

- Applicant must come in person.

- Malaysian Birth Certificate will only be returned back to the applicant once it is received with endorsement from JPN.
