1 copy of Passport Application Form (IM.42-Pin.1/97) – can be downloaded at https://www.imi.gov.my (please print both side in one page).

3 Photographs passport size with white background

Size: 3.5 cm x 5.0 cm

Applicants are required to observe the following requirements:-

1. Background must be white and without shadows.  Colored backgrounds will not be accepted;
2. Applicants are required to wear dark clothing, collared shirt that covers shoulder and chest;
3. Applicants who wears scarf or hijab are required to wear dark-colored scarf or hijab which does not cover the face;
4. Look directly at the camera with an open eyes, mouth closed and both ears visible;
5. Do not wear a cap/hat and hair should not cover the face;
6. Tangled earing are not allowed;   
7. Do not wear contact lenses and glasses

Payment

Mode of Payment : Cash Only

Fee:   AZN 100.00 for adult (12 years old and above)

     AZN 50.00 for below 12 years old

Validity of the passport: Five (5) years from the date of issue.

Process Duration: Standard processing time is 3 months. It may take longer than three months due to high number of passport renewal application at Malaysian Immigration Department, Putrajaya.
