Birth/Marriage Registration
REMINDER:
1. Applicants are advised to read the instruction carefully before fill up the form.
2. All application must be in BLOCK LETTERS (Capital Letters) write clearly and neat.
3. Applications which are not clear without Block Letters will not be accepted/ process. This will delay the entire application.
4. Therefore, please ensure the forms are filled up carefully and clearly in BLACK INK.

| Conditions of Application |
| · Both or either party is a Malaysian citizen; · Has obtained Peru marriage certificate from Registry of Marriage (ROM) Peru; · Application form (JPN.KC 06) cannot be downloaded. It is available at the Consular Section of the High Commission; · Submission of application: (Monday to Thursday - 8.00 am to 12.00 pm); (Friday - 8.00 a.m to 11.30 a.m) |
| Application Procedures |
| · Both parties are required to be present during registration at the Embassy; · Malaysian whose marriage was solemnised by the Registry of Marriage (ROM) Peru should report their marriage at the Embassy of Malaysia within the period of six months from the date of marriage.If registration is made after six months, a penalty of USD 50.00 would be charged for the first year and USD 20.00 for each subsequent year. All payments must be made in US Dollar. |

| · Application form (Borang JPN.KC 06) duly-filled 2 Sets · Malaysian identity card (Original & 2 photocopies) · Peru permanent resident card/Alien card (Original & 2 photocopies) · Birth certificates of both parties (Original & 2 photocopies) (For Birth Certificate other than Malaysia, please provide an english translation and legalize by MFA Peru) · Peru marriage certificate (Please provide and English translation and legalize original and translation by MFA Peru) (Original & 2 photocopies) · For divorcee, please produce divorce certificate / decree nisi/ solvency of marriage certificate from Wisma Putra (Original & 2 photocopies) · Passport of both parties (Original & 2 photocopies) · Recent passport-sized coloured photograph (Two(2) each) · Registration fee USD 10.00 (cash only deposit to Embassy bank account) · Proof of residence in Peru (Dependant Pass,S-Pass,Employment Pass Work Permit/Long-term pass) (Original &2 photocopies) |
Note:
· Submission of application(Borang JPN.KC 06): Monday to Friday (8.00 am to 12.00 pm)
· Collection of certificate (JPN.KC 07) is after three(3) working days: 2.30 pm - 4.00 pm;
· {All photocopies of documents must be in A4 size paper;
· Application without sufficient supporting documents will not be entertained.
- Registration of Birth
Malaysians born abroad can be registered at any Malaysian Mission on the conditions :
- Father of the child is a Malaysian Citizen
- Registration is made within one (1) year from the date of birth
- Date of birth/age
- Address in home country
- Address of relative that can be contacted
Register the birth at the nearest Malaysian Mission. Addresses of Malaysian Missions is available in the main page of this website. Apart from bringing the child to the
Mission, documents required for registration are as follows (original and copy) :
- Passports of parents
- Identification Cards of parents
- Birth Certificates of parents
- Birth Certificate of child and proof of marriage (registered in the country of marriage or Malaysia)
- Acknowledgement letter from the hospital where the child was born
Form W (letter for birth abroad) will be issued by the Mission. A copy will be sent to the National Registration Department of Malaysia to be recorded.
Note: For registration made beyond the period of one (1) year after the birth, process 1 - 2 still applies. The Mission will forward the application to the National Registration Department of Malaysia for approval. Feedback of the application (approved or otherwise) will be notified to the applicant.
