APPLICATION FOR MALAYSIAN PASSPORT
- Applicant is a holder valid Malaysian International Passport;
- Applicant possesses long-term work permit (over 6 months) from Peru, Colombia, Panama and Bolivia
- Children 12 and above must possess MyKad.
A. Required Documents
- Malaysian International Passport;
- MyKad - original and one (1) photocopy of both sides; and
- Permanent Status ID / valid work permits.

- Re A. (Required Documents) above, please kindly bring the original document and one (1) photocopy of each.
- Please avoid white-coloured clothing.
- Please ensure natural hair colour is maintained for photo-taking at the counter during the application.
B. ADDITIONAL REQUIREMENTS FOR APPLICANTS BELOW 18 YEARS OF AGE
- Birth certificate - original and one (1) photocopy;
- Borang W (Born Overseas) - original and one (1) photocopy;
- MyKad - original and one (1) photocopy of both sides; and
- Either parent's MyKad/National Registration Identity Card - original and one (1) photocopy.
C. ADDITIONAL REQUIREMENTS FOR NEW-BORN BABY
- Father's MyKad/National Registration Identity Card - original and one (1) photocopy of both sides;
- Father's Malaysian Passport - original and one (1) photocopy;
- Mother's MyKad/National Registration Identity Card - original and one (1) photocopy;
- Mother's Malaysian Passport - original and one (1) photocopy;
- Both Parents' DNI ID - original and one (1) photocopy; and
- Parent's Marriage Certificate - original and one (1) photocopy.
D. PASSPORT RENEWAL FEE
ADULTS
1) RM 200 (will be converted to USD or PEN using the daily rate)
NOTE:
- Applicant should be present personally to submit his / her application.
- Submission Hours: Mon-Fri; 9.00 am - 12.00 pm.
