NOTE:
i. For baby's FATHER is Malaysian citizen, need to fill in the application form "Borang D" and the Embassy will proceed to issue the Birth Certificate "Borang W" on the same day base on completed supporting documentation. If it is registered to the Embassy over 1 year, the application needs to refer to Jabatan Pendaftaran Negara (JPN) for approval.
ii. For Baby's MOTHER is Malaysian citizen and father is foreign citizen, need to fill in the application form "Borang B", Lampiran A.15(2), and Lampiran A-WN, and needs to be processed in Jabatan Pendaftaran Negara (JPN) for approval.
iii. Both of parents and the new born baby must be present at the Embassy of Malaysia.
iv. Kindly bring along all the original documents and to provide two (2) photocopies of each documents in A4 size.

| 5. | Parents Birth Certificate (original and 2 photocopies)

| 6. | Parents Passport (original and 2 photocopies of page with personal details and page with Russian visa) |
| 7. | Registration Slip of parents (original and 2 photocopies)

| 8. | Russian Immigration card (original and 2 photocopies)

| 9. | Letter (Spravka in English) from university, college or school, confirming the status as a bona fide student if parents on Student Visa (Original and 1 photocopy)

| 10. | Letter (Spravka in English) from company confirming the status as an employee if parents on working visa (Original and 1 photocopy)

| 11. | Father's Malaysian Identity card and Passport (original and 2 photocopies)

| 12. | Mother's local identity card and passport.

| 13. | Marriage certificate of parents (original and 2 photocopies)

| 14. | Fee: Form D, JPN WN02 and Borang W (W.N.23A-PIN.2/81) – RM30.00 equivalent to the foreign currency exchange

NOTE:
i. Processing takes ten (10) working days.
ii. The marriage must be registered with the Embassy within 6 months from the date of marriage. Any registration made after 6 months of marriage is considered as late registration. The fee for late registration is RM100.00 for the first six months and RM50.00 for the following every six months.
iii. The marriage can also be registered in person at any Jabatan Pendaftaran Negara Office in Malaysia within six (6) months of arrival in Malaysia.
iv. For Muslim marriages solemnized in Russia, registration must be done with the Jabatan Agama Islam/ Islamic Religious Department in the applicant’s local area of residence in Malaysia. The Certificate of Conversion and the Marriage Certificate must be translated by a notary public in Russia and followed by an authentication by the Russian Ministry of Justice and the Russian Ministry of Foreign Affairs.
v. For non-Muslim marriages solemnized in Russia, the Marriage Certificate must be translated by a notary public in Russia and followed by an authentication by the Russian Ministry of Justice and the Russian Ministry of Foreign Affairs.
