1 copy of Emergency Certificate Application Form (Imm.41-Pin.2/86) - can be download at www.imi.gov.my or obtained at the Embassy

 

2.

1 copy of Borang Soal Selidik Kehilangan Pasport Malaysia (Im.42c) - can be obtained at the Embassy

 

3.

1 copy of Surat Akuan - can be obtained at the Embassy

 

4.

Police Report

 

5.

Confirmed Return Ticket to Malaysia

 

6.

Malaysian Birth Certificate (original and 2 photocopies)

 

7.

Malaysian Identity Card (original and 2 photocopies)

 

8.

Copy of Passport (if available)

 

9.

Copy of Russian Visa (if available)

 

10.

Russian Immigration card (original and 2 photocopies) (if available)

 

11.

Registration Slip (original and 2 photocopies)

 

12.

Letter (Spravka in English) from university, college or school, confirming the status as a bona fide student if applicant is on Student Visa (Original and 1 photocopy)

 

13.

Letter (Spravka in English) from company confirming the status as an employee if applicant is on working visa (Original and 1 photocopy)

 

14.

4 recent colored photograph with white background, size 3.5 cm X 5.0 cm

 

15.

Fee: RM50.00 equivalent  to the foreign currency exchange

 

Mode of payment: Cash in Russian Ruble

 

 

NOTE:

 

An Emergency Certificates is not a passport and it is solely used to facilitate the holder to return to Malaysia only.

 

After issuance of the Emergency Certificate, applicant must obtain exit visa in order to leave the territory of the Russian Federation.
