| 4. | Malaysian Birth Certificate (original and 2 photocopies) |
| 5. | Proof of Malaysian Citizenship (if any) |
| 6. | 3 colored Photographs, size 3.5 X 5.0 cm |
| 7. | Chinese Passport or Certificate of Chinese Citizenship (original and 2 photocopies) |
| 8. | Fee: RM20.00 equivalent to the foreign currency exchange
