* For couples where both parents or the father are Malaysian citizens, the following is a list of requirements:

Documents to be submitted

1.

An application Form D

(obtainable only from the Embassy/Consulate General of Malaysia) 

 

Two (2) colour photographs of the applicant with a white background.

Biometric size, 3.5 cm x 5.0 cm

2.

The original birth certificate of the children issued by the Ministry of Interior Turkiye (please refer to Nüfus Müdürlüğü– Civil Registry Office). It needs to be translated into English and notarised by the local authority.

Malaysian Identity Card of both parents (original and one photocopy) – For Malaysian citizen

4.

Original and one copy of parent's Passport

5.

Valid Turkish Residence Permit

(if you have lived in Turkiye for more than 90 days upon your arrival)

Original and copies of Marriage Certificate (If the Turkish Authority issues the certificate, it must be attested by the Ministry of Foreign Affairs, the Republic of Turkey, in Ankara / If the Malaysian Authority issues the certificate, it must be certified by the Ministry of Foreign Affairs, Malaysia, in Wisma Putra, Putrajaya)

Note: 

  • The document submission must be completed before one year of the applicant (baby);
  • Birth registration will only be accepted at the Embassy/Consulate General's Consular counter. Application by post will not be accepted;
  • Up to 3 working days or more to process each application;
  • All photocopies are required to be made on A4-size paper;
  • The parents and the applicant (baby) must be present during the registration in the Consulate Office.

* For submission more than one (1) year from the date of birth, the application needs to be referred to Jabatan Pendaftaran Negara (JPN) for approval of the issuance of ‘Borang H’.
