DOCUMENT CHECK LIST FOR eVISA MEV APPLICATION

AS OF JANUARY 31^st, 2023

MULTIPLE ENTRY VISA (MEV)

1. FLY & CRUISE

1. Valid passport (at least more than 6 months from the date of arrival in Malaysia);
2. Studio passport photo with white background;
3. Cover/ explanation/ application letter;
4. Travel itinerary

• Cruise : Cruise itinerary;
• Fly       : Flight schedule;

5. Confirmed return travel ticket

• Cruise : From/ to Malaysia via gazetted entrance;
• Fly       : Flight ticket

6. Cruise/ accommodation proof payment/ receipt;
7. Latest bank statements (3 consecutive months with cumulative ending balance ≥ USD500.00); and
8. Other relevant documents that can support the application.

2. WEDDING TOURISM

1. Valid passport (at least more than 6 months from the date of arrival in Malaysia);
2. Studio passport photo with white background;
3. Cover/ explanation/ application letter;
4. Travel itinerary;
5. Confirmed return travel ticket (1^st entry);
6. Accommodation proof payment/ receipt (1^st entry);
7. Latest bank statement (3 consecutive months with cumulative ending balance ≥ USD500.00); and
8. Other relevant documents that can support the application.

3. BUSINESS

1. Valid passport (at least more than 6 months from the date of arrival in Malaysia);
2. Studio passport photo with white background;
3. Cover/ explanation/ application letter;
4. Business travel itinerary (6 months);
5. Confirmed return travel ticket (1^st entry);
6. Accommodation proof payment/ receipt (1^st entry);
7. Latest bank statement (3 consecutive months with cumulative ending balance ≥ CNY100,000.00);
8. Invitation letter from government/ private agency/ sponsor/ company;
9. Certificate of Malaysia registration company for private agency/ sponsor/ company;
10. Dispatch letter;
11. Certificate of Local company registration with translation; and
12. Other relevant documents that can support the application.

4. MEDICAL

1. Valid passport (at least more than 6 months from the date of arrival in Malaysia);
2. Studio passport photo with white background;
3. Cover/ explanation/ application letter;
4. Confirmed return travel ticket (1^st entry);
5. Accommodation proof payment/ receipt (1^st entry);
6. Latest bank statement (3 consecutive months with cumulative ending balance ≥ CNY100,000.00);
7. Reference letter from registered medical centre,
8. Appointment letter/ schedule; and
9. Other relevant documents that can support the application.
