EMERGENCY CERTIFICATE APPLICATION
Emergency Certificate is a one-way travel document issued by the Embassy of Malaysia in Abu Dhabi to facilitate Malaysian citizens in United Arab Emirates (UAE) who do not have a valid passport (either expired, lost or stolen) to return to Malaysia. Those who have obtained an Emergency Certificate from the Embassy are highly encouraged to take a direct flight from UAE to Malaysia.
Application Requirements:
- Application must be done in person at any of the missions.
- Applicants who lost their passports will be required to submit a police report and fill in a questionnaire (IM.42C) on the loss of their passports.
Required documents:
- Passport Application form (click IM. 42 – Pin. 1/97) please print on both side of an A4 paper;
- Parents’ consent Form for applicant 18 years old and below (click IM 42A - Pin. 1/93);
- Two (2) passport size photos (latest photos, white background, dark shirt, without glasses, without accessories, hair must not cover the forehead and ears);
- Original and a copy of Malaysian Passport (only for expired passport case)
- Original and a copy of Police Report (for lost passport case) and will be required to fill in a questionnaire (IM.42C) on the loss of their passports;
- A copy of Malaysian Passport (for lost passport case);
- Original and a copy of Malaysia Identity Card (IC);
- Original and a copy of Birth certificate/From W/Form H/Certificate of Citizenship, if any; and
- Original and a copy of valid UAE Residency Visa / Emirates ID card (for UAE Residence).
Fees: AED 50.00 (cash only)
Processing Time: Three (3) working days.
Note:
Kindly bring along all the original documents and to provide two (2) photocopies of each document in A4 size.
